About this file
This document is Amendment No. 2 to AHCA Contract No. EXD100, an agreement between the State of Florida's Agency for Health Care Administration (the "Agency") and Shutts & Bowen, LLP (the "Vendor"). The amendment was executed in April 2020 and involves a contract renewal between these two parties.
The amendment extends the contract's duration, modifying the original contract term to now run from July 1, 2022, to June 30, 2025, inclusive. Specifically, the parties are exercising the second renewal option for a twelve-month period beginning July 1, 2024, in accordance with Section 287.058 of Florida Statutes. The document indicates that all previous provisions of the contract not in conflict with this amendment remain in effect, and the amendment becomes valid only after being signed by authorized representatives from both the Agency and the Vendor, which occurred between February and April 2024.
