FM 442667-1-32-01 Contract No. CA015

SUPPLEMENTAL AMENDMENT #4 (BASIC SERVICES)

PROJECT DESCRIPTION AND PURPOSE

This is a supplement to the Dupont Bridge Replacement PD&E Study located in Bay County. The SR 30 (US 98) project limits are from the Oak Shore Drive intersection in the City of Parker to the Tyndall Air Force Base (TAFB) Bonita Bay Outdoor Recreation Center intersection, a distance of approximately one mile.

The purpose of this Supplemental Amendment is for the CONSULTANT (RS&H) to provide professional engineering services for the preparation of a modified set of Phase II plans, develop information required for the submittal of permits for the currently preferred concept (Alternative West 1), and assist the DEPARTMENT (FDOT) in the development of the Design-Build Request for Proposal (RFP).

1. PROJECT GENERAL TASKS (LS)

a. Contract maintenance.

b. Project Manager meetings.

2. ROADWAY ANALYSIS AND PLANS (LS)

a. Typical section package (TSP) has been prepared as part of the PD&E. Time is not included in this SA to develop a new TSP.

b. Develop the horizontal/vertical master files, calculations, and plan sheets for the 60% submittal. Design files will be prepared to an approximate 60% and plan sheets to 45% completion.

c. Develop the intersection designs at Oak Shore Drive and the TAFB Bonita Bay Outdoor Recreation Center

d. Temporary traffic control analysis of key locations to ensure the proposed concept can be constructed, with emphasis on issues that may need to be addressed in the RFP, access to adjacent parcels and potential TCE's. Does not include plan development, just typical sections.

e. Prepare design documentation as needed.

f. Develop and update the LRE as required.

g. Attendance at meetings by required project staff.

3. DRAINAGE ANALYSIS AND PLANS (LS)

a. The proposed stormwater management system will utilize the pond location that is identified in the Pond Siting Report to accommodate the preferred alternative (West-1). The pond site is located on the north (Parker) side of the bridge. As there will be no stormwater retention ponds on the TAFB side of the bridge, compensatory treatment will be incorporated in the pond on the north side of the bridge.

b. The drainage systems will be a closed system along the landfall areas adjacent to the twin bridges. These systems will be designed to a Phase II level. The Dupont Bridge will have a

c. Develop the drainage master files and calculations and for 60% submittal. Design files and plan sheets will be prepared to an approximate 45% completion. Calculations will include the design of cross drains, ditches, retention ponds and on-site conveyance system.

d. A Pond Siting Report (PSR) was prepared during the PD&E. Additional pond siting is not required.

e. FEMA FIRM Panel No.12005C0426H indicates that the project is located in a 100-year floodplain. However, due to its proximity to St. Andrew Bay, any minor encroachment into the base flood plain is considered tidal. As a result, floodplain compensation will not be required..

f. Prepare design documentation as needed.

g. Attendance at meetings by required project staff.

4. UTILITIES (LS)

a. Data collection and coordination with UAO's on potential conflicts with the proposed design.

5. ENVIRONMENTAL PERMITS (LS)

a. Limits of permitting for this SA are the currently designated project limits.

b. Wetland Delineation and Jurisdictional Approvals

c. Prepare, submit, and acquire FDEP, USCG and USACE permits.

d. The DEPARTMENT will provide compensatory wetland mitigation in accordance with Section

373.4137, Florida Statutes. The Consultant will provide the UMAM worksheets. Mitigation design will not be performed

e. Based on the PD&E, it is expected that the project will receive a designation of May Affect, Not Likely to Adversely Affect from the FWS and FWC. Therefore, while an updated protected species survey will be conducted in conjunction with the wetland delineation, protected species permitting is not included with this scope. Should protected species permitting become warranted, an additional scope and fee will be provided.

6. STRUCTURES (LS)

a. A BDR memo with limited bridge alternatives has been prepared in the PD&E phase. The BDR memo evaluated basic bridge geometry, span lengths, typical sections, superstructure types, substructure types, foundation alternatives, and cost estimates. No modifications to the existing document.

b. Utilizing the information contained in the BDR memo RS&H will develop one set of bridge plans (size 8.5'' x 11.0") in support of the USCG permit application. The bridge plans will include the following information: (1) Location Map - 1 Sheet; (2) Plan and elevation sheets for final proposed bridge - 6 sheets; (3) Plan and elevation showing navigation channel - 1 sheet; (4) Typical section of final proposed bridge section - 1 sheet; and (5) Fender system layout plan (if necessary) – 1Prepare structures exhibits or concept plans.

Supplemental Amendment for 60% Design

FM 442667-1-32-01 Contract No. CA015

c. Develop limits and profiles of retaining walls. No structural design, and plans production effort will be limited to preliminary geometry and wall control drawings

d. The storm water analysis will simulate a hurricane storm surge with the 2-D coupled SWAN+ADCIRC model to develop 100- and 500-yr wave and hydrodynamic conditions at the bridge crossing. In turn, these conditions will be used to help develop the wave crest envelope, wave forces, general contraction and local scour at the bridge piers, and abutment protection. A Bridge Hydraulic Report will document the methods and results.

7. SIGNING & PAVEMENT MARKING ANALYSIS AND PLANS (LS)

a. Pavement markings will be provided for the landfall areas adjacent to the bridge. Basic plans sheets covering this area will be developed to a modified Phase II level. Only critical annotations will be provided. Develop the master files for the 45% submittal. Design files will be prepared to an approximate 45% completion.

a. Establish new Horizontal and vertical control as needed using the already established control from previous work performed.

b. Using the existing alignment reset all positions along that survey baseline. This information will be used to perform data gathering for north and south of the existing bridge and along the proposed new route north of the bridge.

c. New data gathered will be all above ground features and terrain data to complete plan and DTM modeling. The terrain data/DTM the pond site and outfall. Once completed SSMC will perform check cross sections to prove data gathered.

d. Utilities will be located and structures for sewer and drainage will be fully detailed to include SUE test holes at conflict locations.

e. A Bathymetric survey will use a fathometer to gather data on the sub-aqueous floor of the bay with the limits being 500 feet east and 800 feet west of the existing bridge.

f. The existing bridge will be detailed to show the existing sub structure.
