About this file
This Task Assignment Form (Attachment B) under DEP Contract No. WQ034 is an indefinite delivery vehicle issued by the Florida Department of Environmental Protection (DEP) to the St. Johns River Water Management District. The assignment was executed on January 13, 2017, with DEP Task Manager Misty Alderman overseeing performance and contractor representative Jessica Beach, P.E., managing the work for the St. Johns River Water Management District.
Task Assignment No. 2, valued at $20,000 on a fixed price basis, requires the contractor to prepare a Quality Assurance Project Plan (QAPP) or Sampling and Analysis Plan and submit an initial background sampling report documenting baseline conditions at the Lake Apopka North Shore project site near Apopka, Florida. Deliverables include the QAPP and background sampling report in Word or PDF format. Background samples must be collected at three canal locations at two depths (0.5 meters below surface and near the canal bottom at targeted suctioning depth), with laboratory analysis of results included in the initial report. The period of performance extends from task assignment execution through June 30, 2017. Funding is provided under Org Code 137220400000, E.O. 1751005, with the full $20,000 allocated in fiscal year 2016. Performance measures require DEP Task Manager verification that deliverables meet specified requirements, with financial consequences including no payment for unsatisfactory work and potential contract termination for failure to perform.

Task Description: The overall objective of this project, with multiple tasks, is to continue the final phase of the continuous controlled cavitation process, which will remove multiple metric tons of Phosphorus from the effluent at the bottom of the canal and return the water from the slurry at Phosphorous levels in line with state mandated levels for Lake Apopka. The removed Phosphorous material will be left to compost on District property and tested to determine if the removed phosphorous rich material is reusable for agricultural use (soil amendment) in the State of Florida. The Contractor will treat a source of suctioned sediment and water using the controlled cavitation technology. The project site is on District-owned property in the west marsh area of the Lake Apopka North Shore near Apopka, FL. Collection and analyzation of samples for the background condition, inflows and outflows of the controlled cavitation technology with separate samples for the outflow decant water and resulting slurry will occur. This project includes preparation of the site, site work, machine operation, suction harvesting of suspended floe, removal of treated mud from the canal to the dispersion site for composting, weekly water sampling and monthly reports.
TASK 2 DESCRIPTION: Initial Report of Samoling Protocols and Background Sample Results: Once the system has been commissioned, the Contractor must complete and submit a Quality Assurance Pro;ect Plan (OAPP) or Sampling and Analysis Plan for monitoring. The monitoring plan will specify the samoling locations samQ.ling instruments. and P,arameters to be sampled and will need to be approved by the DEP Task Manager before sampling can commence. The background samples will be taken at 3 locations in the center of canal at two different depths in each location. The first depth sampled will be 0.5 meters below the surface: the second deoth will be near the bottom of the canal at the tarcieted suctioning depth. Samples will be analvzed by a lab and results for each individual samD!e will be included as part of the initial report.
Deliverables: 1 ! OAPP in Word or PDF format; 2) Initial background sampling report in Word or PDF format
Performance Measures: The DEP Task Manager will review the submitted initial background sampling report and OAPP to verify that they meet the soecifications as described in Task Number 2.
