About this file
This document is Amendment No. 5 to DEP Contract No. LAB028, a services contract between the Florida Department of Environmental Protection (FDEP) and TestAmerica Laboratories, Inc., a laboratory located in Tallassee, Florida. The contract manager is Kate Brackett. The contract was originally executed on December 12, 2007, with an end date of December 11, 2012. The amendment was executed in 2011 and includes a Separation of Duties Notice dated May 23, 2011, prepared by Contracts Office representative Debbie Bates.
The contract provides overflow laboratory services for inorganic and organic samples. Amendment No. 5 increases the contract funding from $340,522.20 to $370,522.20, representing an increase of $30,000.00, effective July 1, 2011. The amendment also adds a new requirement that the contractor enroll and participate in the federal E-Verify Program for Employment Verification within thirty days of the amendment's effective date, and requires all subcontractors performing work under the contract to enroll in E-Verify within ninety days of their engagement or the amendment's effective date, whichever is later. The contractor must maintain records of participation and compliance and make such records available to the Department upon request. Non-compliance with the E-Verify requirement is treated as a material breach of the contract.
