About this file
This is Amendment No. 16 to Agency Term Contract No. GC873, a state contract between the Florida Department of Environmental Protection (FDEP) and MDM Services, Inc. The original contract was effective February 20, 2014, and this amendment extends the contract term. The contract is a petroleum restoration program vehicle through which the state procures site response action services related to petroleum contamination.
The amendment extends the contract for an additional 6-month period beginning August 14, 2024, and ending February 14, 2025, under the same terms and conditions as the original contract. No new Purchase Orders or Task Assignments may be issued after the extended term concludes; however, any Purchase Orders or Task Assignments issued during the renewal or extended term may continue for up to twelve months beyond the extended term for completion purposes. The contractor attests under penalty of perjury that commodities or services offered have not been produced by forced labor and acknowledges that the Department may terminate the contract should the contractor be placed on the force labor vendor list. All other contract terms and conditions remain in effect, with amendment provisions controlling in the event of any inconsistencies between the original contract and this amendment.

NOW, THEREFORE, the parties agree as follows:
1. The Contract is extended for a 6-month period, pursuant to section 287.057(13), F.S., to begin August 14, 2024, and remain in effect until February 14, 2025.
