About this file
This is Amendment No. 12 to Agency Term Contract No. GC873, a state contract between the Florida Department of Environmental Protection (DEP) and MDM Services, Inc. The original contract was effective February 20, 2014, and covers petroleum contamination site response action services under the Petroleum Restoration Program.
The amendment modifies the Scope of Services, specifically Section 12 (Debris, Waste and Product Removal and Disposal) under Attachment A (V2.0). The revision establishes procedures for handling volume overages in soil delivery: when delivery weight tickets indicate an overage exceeding the volume approved on a Purchase Order, notification to the site manager via a Request for Change is required within two business days, provided the soil overage does not exceed 20 percent of the total volume authorized under the Purchase Order. All other terms and conditions of the contract remain in effect. The amendment was signed on August 5, 2022, by Michael M. Alexander, Vice President of MDM Services, Inc., and John Truitt, Deputy Secretary of the Florida Department of Environmental Protection.
