Amendment No. 13 to DEP Agency Term Contract No. GC761
This document is the thirteenth amendment to an agency term contract between the Florida Department of Environmental Protection (DEP) and Groundwater & Environmental Services, Inc. The contract was originally effective February 17, 2014, and addresses petroleum contamination site response action services. The amendment was executed on August 15, 2022, by Ben LaMonte, Sr. Operations Manager for the contractor, and John Truitt, Deputy Secretary for the Florida DEP.
Amendment No. 13 modifies the Scope of Services, specifically updating Section 12 (Debris, Waste and Product Removal and Disposal) of Attachment A (V2.0) regarding soil removal procedures. The amendment establishes that when delivery weight tickets indicate an overage of the volume approved on a Purchase Order, and the additional soil volume does not exceed 20 percent of the total volume authorized under the Purchase Order, the contractor must notify the site manager via a Request for Change within two business days of the date the overage was transported from the site. All other contract terms and conditions remain in effect, with the amendment's provisions controlling in cases of any inconsistency with the original contract.
