SUBJECT: Request for Proposal Number CMS-2012-ESRD-FFPCOM
NOTICE: THIS REQUEST FOR PROPOSAL IS BEING ISSUED PENDING AVAILABILITY OF FUNDS
Ladies and Gentlemen:
You are invited to submit a proposal in accordance with the requirements of Request for Proposal (RFP) Number CMS-2012-ESRD-FFPCOMP to provide services for and operation of End State Renal Disease Organization (ESRD) in accordance with Title XVIII of the Social Security Act as amended by Section 1881(c).
This solicitation is for seven separate Network Area Competitive RFP’s. Attached to this letter is the current ESRD Network Map and the Geographic Areas showing the states/regions represented by each Network. These nine separate competitions are for Network Areas 1, 2, 4, 7, 10, 11, 13, 16 and 18.
If you are proposing on more than one Network Area, you must submit a separate proposal for each Network Area.
There is a separate Non-Competitive Firm-Fixed Price RFP for Renewal ESRD Networks 3, 5, 6, 8, 9, 12, 14, 15 and 17.
Your attention is directed to the additional information provided below:
1. Eligibility Offerors submitting proposals in response to this solicitation must meet the definition of an ESRD in accordance with the Social Security Act as amended by Section 1881(c). Proposals received by offerors who do not qualify as an ESRD will not be evaluated.
2. Pre-Bidders Conference The End Stage Renal Disease Network Bidders Conference will be held on Thursday, June 21, 2012 from 12:00 – 1:30 PM (local prevailing time-Baltimore). Meeting participation may occur via Adobe Connect (Webinar). Pre-registration is not required.

3. Questions The due date and time for submission of RFP Questions is June 26, 2012 at 5 p.m. local prevailing time (Baltimore, MD). Questions must be submitted in accordance with Section L.11. Questions must be submitted by email to: esrd_nw_sow_questions@cms.hhs.gov Offerors are advised that verbal questions will not be accepted. Please identify the RFP you are responding to when you submitting your proposal.
4. RFP Amendments Please be advised that any amendment to this RFP will be issued and posted electronically on Fed Biz Opps. It is strongly recommended that offerors monitor Fed Biz Opps regularly for information related to this RFP.
5. Due Date for Proposals Proposals must be received on or before August 2, 2012 at 2 p.m. local prevailing time (Baltimore, MD). Delivery of proposals must be made so that the proposal is received at the designated place (7500 Security Boulevard, Mail Stop B3-30-03) not later than the date and time. If delivery is by hand, please make prior arrangements with Brian Hebbel by contacting him at 410-786-5159. Refer to Section L for complete proposal submission information.
6. Renewal ESRD and Competitive Offerors This RFP pertains to Firm Fixed Price type contracts. Offerors should ensure that the appropriate instructions are followed when developing and submitting proposals.
7. Request for Information Concerning the RFP Request for information concerning the RFP should be directed only to the undersigned Division Director who may be reached at 410-786-5159 or Brian.Hebbel@cms.hhs.gov . Discussions with individual outside of the contracts office may result in rejection of a prospective offeror’s proposal.
8. Your proposal must clearly state the RFP number and the Network Area Number.
