This structure is a one span precast, prestressed concrete deck beam bridge carrying Bull Valley Road over Boone Creek. It was originally constructed in 1968 and repaired in 2000 with new fascia beams and railings.
It has a Sufficiency Rating of 46.7 and is eligible for matching funds. The original deck beams are over 50 years old.
Overall, this structure is in poor condition. The concrete wearing surface constructed in 2000, exhibits some hairline cracks, which may have formed shortly after construction. There is one (1) minor crack in the southwest wingwall, and a spalled off corner at the south end of the east abutment. We noted that the cracks, delaminations, and spalls on the underside of five deck beams are getting larger since our last inspection. This required dropping the superstructure rating to 4, which may trigger an IDOT Load Rating Inspection. The PPC Deck Beams are near the end of their service life.
This bridge should be programmed for replacement within the next 4 to 8 years. It may be possible to reuse the abutments which are in satisfactory condition.

5. Prime Farmland (BLRS Manual Section 20-10)
a. Does the project involve Prime Farmland?
YesNo
b. Does the project require more than 3 acres/mile (0.75 hectares/kilometers), 10 acres (4 hectares) for a non-linear improvement, or the project ROW is not contiguous to the existing ROW?
YesNo
c. The project requires consultation with the Natural Resource Conservation Service (NRCS). Form AD-1006 has been completed and submitted to the local office of NRCS.
The impact of this project on farmland conversion has been evaluated in accordance with the requirements of the US Natural Resources (NRCS). The project will cover 3 acres or less of farmland per mile (0.75 hectares or less of farmland per kilometer) and the conversion will not result in more than minor impacts. Accordingly, the project conforms to the general form AD-1006 prepared by NRCS. Therefore, further coordination with NRCS on this project will not be necessary.
6. Floodplain Encroachment (BLRS Manual Section 20-7)
a. Does the project work cross or encroach upon a 100-year floodplain, including a regulatory floodway?
YesNo
Summarize the location hydraulics study, regulatory floodway restrictions, the effect of any encroachment (including a comparison between existing and proposed conditions) and the effect of the over-the-road flow on the proposed transportation facility. Include available floodplain attachment.
Boone Creek is classified as Zone A regulatory floodplain as shown on the FEMA FIRM panels (17111C0205J and 17111C0208J). However, Boone Creek is not studied in the McHenry County Flood Insurance Study (FIS), and no detailed base flood elevations (BFE) are available. A review of the FEMA Flood Insurance Map indicates that no regulatory floodway is designated at this location. A HEC-RAS model of the crossing was completed to determine the water surface elevations for design.
StreamStats indicates that the total Boone Creek watershed is 15.62 square miles. The discharges obtained from StreamStats were used for the hydrology at this location. The soil near Bull Valley Road primarily consists of sand loam according to the soil boring and the geotechnical report. A HEC-RAS model of the crossing was completed to determine the water surface elevations for design.
The existing Bull Valley Road bridge over Boone Creek was originally constructed in 1968. In 2000 the exterior deck beams of the bridge were replaced, a waterproofing membrane was added and a 2”-4” concrete wearing surface was added. The existing structure consists of a one-span prestressed concrete deck beam structure with a total length of 62 feet and a deck width of 27 feet. The bridge is positioned at a 15-degree angle skew relative to the road. The bridge has approximately 2:1 side and foreslope on both sides at the abutments.
The existing bridge will be replaced with a single-span prestressed concrete beam and concrete deck, which will have a length of 61.4 feet face to face of abutments and a width of 54 feet. The new bridge will maintain the same 15-degree skew relative to the road, and it will retain the 2:1 slope abutment on both sides, like the existing design.
The proposed bridge with a lowered low chord by 1.2 feet, satisfies the design criteria established by IDOT and IDNR-OWR. The proposed improvements will not result in an increase in headwater elevation. There will be no

b. Mobile Source Air Toxics (see BDE 52-06)
The ADT is projected to be less than 140,000 vehicles per day. Therefore, MSAT analysis is not required.
This project is of a type qualifying as a categorical exclusion under 23 CFR 771.117(c), or exempt under the Clean Air Act conformity rule under 40 CFR 93.126, and, as such, a Mobile Source Air Toxics analysis is not required.
c. Construction-related Particulate Matter
Demolition and construction activities can result in short-term increases in fugitive dust and equipment-related particulate emissions in and around the project area. (Equipment-related particulate emissions are usually insignificant when equipment is well maintained). The potential air quality impacts will be short-term, occurring only when demolition and construction work is in progress and local conditions are appropriate.
The potential for fugitive dust emissions typically is associated with building demolition, ground clearing, site preparation, grading, stockpiling of materials, on-site movement of equipment, and transportation of materials. The potential is greatest during dry periods, periods of intense construction activity, and during high wind conditions.
The Department's Standard Specifications for Road and Bridge Construction include provisions on dust control. Under these provisions, dust and airborne dirt generated by construction activities will be controlled through dust control procedures or a specific dust control plan, when warranted. The contractor and the Department will meet to review the nature and extent of dust-generating activities and will cooperatively develop specific types of control techniques appropriate to the specific situation.
Techniques that may warrant consideration include measures such as minimizing track-out of soil onto nearby publicly-traveled roads, reducing speed on unpaved roads, covering haul vehicles, and applying chemical dust suppressants or water to exposed surfaces, particularly those on which construction vehicles travel. With the application of appropriate measures to limit dust emissions during construction, this project will not cause any significant, short-term particulate matter air quality impacts.
d. Project-level Hot Spot Analysis. Select One
This project is in an attainment area and does not require hot spot analysis.
This project does not meet the definition of a project of air quality concern as defined in 40 CFR 93.123(b)(1).
Due to it not being a new or expanded highway project that has a significant increase in diesel vehicles, it has been determined that the project will not cause or contribute to any localized PM2.5 or PM10 violations or increase the frequency or severity of any PM2.5 or PM10 violations. USEPA has determined that such projects meet the Clean Air Act's requirements without any further Hot-Spot analysis.
This project is in a non-attainment or maintenance area and is a project of air quality concern. Therefore, a qualitative hot spot analysis is required. (14d) Attach Hot-Spot Analysis
