A MOT Plan shall conform to, unless otherwise noted in the Broward County Minimum Standards, the latest editions of the Florida Department of Transportation (FDOT) Design Standards 600 Series and the manual on Uniform Traffic Control Devices (MUTCD). An approved MOT Plan and copy of the permit, if issued, must be onsite at all times. The MOT is valid for the duration of the permit or completion of the project, whichever is sooner. Once the MOT is approved by BCTED, the permitted shall be solely responsible for the installation and maintenance of the approved work zone traffic control devices throughout the length of the project.
16.1 MAINTENANCE OF TRAFFIC AND PROTECTION OF WORK SITE
Contractor shall furnish and install all items necessary in order to re-direct, protect, warn and/or maintain existing vehicular and/or pedestrian traffic during the course of construction, including, as necessary, flaggers and off-duty Police Officers/Sheriff's Deputies. Items to be provided shall include, but not be limited to, barricades, signs, arrow boards, warning/flashing lights, pavement markings and traffic signal modifications.
Prior to beginning work, Contractor shall prepare a "Maintenance of Traffic Plan" drawn to a suitable scale. This plan shall be submitted to BCTED for review and shall be amended as requested by BCTED. No work shall begin until such time that the "Maintenance of Traffic Plan" has been approved in writing by BCTED.
The "Maintenance of Traffic Plan", provided by the Contractor, shall include provisions for pedestrian and/or school student traffic as well as vehicular traffic. The following are minimum requirements:
a) A safe walking route for all school students within the vicinity of the construction zone shall be maintained during the times students are arriving at or leaving school.
b) If the current walking surface cannot be maintained, then a temporary road-rock 4-foot width walkway shall be created.
c) The safe walk route shall be separated from the construction activity by a 4-foot-high orange construction fence for the entire length of the project or the length of the walk route, whichever is less.
d) All construction equipment around any designated crosswalk shall cease to operate during the times students are arriving at or leaving school.
e) All construction equipment adjacent to a designated walk route shall cease operation during these times unless satisfactorily barricaded from the walk route.
f) In the case that a designated crossing or any portion of the designated walk route cannot be maintained, then the Contractor shall notify the "School Safety Coordinator" at BCTED, (954) 847-2671, a minimum of ten (10) working days prior to closing that route in order that an alternate crossing/route can be established.
g) Contractor shall install any necessary Pavement, Road Rock, Pavement Marking and Signage and/or any Pedestrian Signalization and/or Signal Modification to accommodate an existing or alternate walk route.
h) Contractor shall provide State Certified School Crossing Guards or off duty Police Officers to cross students at any locations other than those previously designated.
i) Contractor may use flag person, only if they are State Certified as a School Crossing Guard.
j) Ten days prior to the beginning of construction, Contractor shall notify the "School Safety
Coordinator" at BCTED, 954-847-2671, to arrange a "pre-construction safety" meeting.
k) Contractor shall be responsible for providing a safe and adequate walking surface for all school children/pedestrians.
l) This safe walking route shall be part of the “Maintenance of Traffic Plan".
m) Contractor, at all times, shall conduct the work in such a manner as to ensure the least obstruction to traffic as is practical. Convenience of the general public and of the residents adjacent to the work shall be provided in a safe and satisfactory manner.
n) Sidewalks, gutters, drains, fire hydrants and private drives shall, insofar as practical, be kept in condition by the Contractor so as to be able to continue functioning for their intended uses. Fire hydrants on or adjacent to the work shall be kept accessible to fire apparatus at all times, and no material or obstruction shall be placed within 10 feet of any such hydrant.
o) Construction materials stored upon the public street shall be placed so as to cause as little obstruction to the general public as is reasonably possible.
p) Streets shall not be closed, except when and where allowed by the BCTED and, whenever the street is not closed, the work must be conducted with the provision for a safe passageway for vehicular and pedestrian traffic at all times. Contractor shall make all necessary arrangements with the BCTED concerning maintenance of traffic and selection of detours required.
q) All existing traffic control devices shall be maintained by the Contractor for the duration of the project. If any signs or signals are damaged or lost during the construction period, such signs and signals shall be repaired or replaced by the Contractor at Contractor ’s expense.
r) When traffic is diverted temporary pavement markings may be required. Existing pavement markings that conflict with the new work zone traffic pattern must be obliterated by sandblasting and/or water jet. Painting over existing markings (black out) is not permitted.
s) Contractor may be required to reposition existing traffic heads in order to maintain
i. traffic flows at diverted intersections. If this should be necessary, Contractor must submit a plan for approval showing the course of work and the planned repositioning. No separate payment for repositioning the existing traffic signal heads will be made.
17. EXCAVATION
a) Bracing and shoring, sheeting, bracing, etc., shall be used by the Contractor as required to support the sides of the excavation and to prevent any movements which can in any way alter the grade of or injure the facility being installed, diminish the width of excavation or otherwise injure or delay the work or endanger personnel, adjacent pavements, or other structures.
b) Safety procedures shall be followed, and adequate protection shall be furnished by the
Contractor to all personnel as required by OSHA and state statute. All sheeting or bracing which is not left in place shall be removed in a manner that shall not endanger the work, personnel or adjacent structures. To prevent trench undermining, proper trenching methods and equipment shall be used, and all personnel and equipment shall be kept at a safe distance. Excavations are not to be left open; all necessary precautions are to be taken to avoid public safety hazards.
c) No separate payment will be made to the Contractor for earthwork, excavation, trenching, furnishing and importing fill and compaction, therefore, this shall be included in the prices bid for the various appurtenances needed to complete the items of work. If trench excavation is five feet deep or greater, bid item “Excavation Safety” will apply for additional excavation and protective system as required by all current federal, state, and local regulations, laws, ordinances, rules, standards and regulatory agency requirements.
d) Contractor shall perform all excavation of every description and of whatever substances encountered. All excavations shall be made by open cut. All existing utilities such as pipes, poles, and structures shall be carefully supported and protected from injury, and in case of damage, they shall be restored at no cost to the County.
e) Work shall be properly sheeted and braced where necessary. Where wood sheeting or certain designs of steel sheeting are used, the sheeting shall be cut off at a level two feet above the top of the installed pipe and that portion below that level shall be left in place. If interlocking steel sheeting, of a design approved by the Engineer of Record is used, it may be removed providing removal can be accomplished without disturbing the bedding or alignment of the pipe.
Any damage to the pipe bedding, pipe, or alignment of the constructed main caused by removal of sheeting shall be cause for rejection of the affected portion of the work.
f) Excavation for appurtenances by the Contractor shall be sufficient to provide a clearance between their outer surfaces and the face of the excavation, or sheeting if used, of not less than 12 inches. Materials removed from the excavation shall be stored and disposed of in such a manner that they will not interfere unduly with traffic on public streets and sidewalks and shall not be placed on private property. In congested areas, such materials that cannot be stored adjacent to the trench or used immediately as backfill, if acceptable, shall be removed to convenient places of storage.
g) All excess material suitable for use as backfill shall be hauled to and used in areas where not enough suitable material is available from the excavation. Suitable material in excess of backfill requirements and material unsuitable for backfill shall become the property of the Contractor and shall be removed from the work and disposed of by the Contractor at its own expense.
h) Unsuitable material shall not be mixed with or allowed to contaminate suitable backfill material by the Contractor .
17.1 TRENCH STABILIZATION
No claims for extras or additional payment will be considered for cost incurred in the stabilization of trench bottoms which are rendered soft or unsuitable as a result of construction methods, such as improper or inadequate sheeting, dewatering or other causes. In no event shall pipe be installed when such conditions exist, and the Contractor shall correct such conditions so as to provide proper bedding or foundations for the proposed installation.
18. DEWATERING
When practical, it is a requirement of these specifications that excavation shall be free from water before pipe, or structures are installed. When not practical, work shall be done as specified by the County’s Project Manager or designee.
Contractor shall provide all necessary pumps, under-drains, well point systems, and other means for removing water from trenches and other parts of the work. Contractor shall continue dewatering operations until the backfill has progressed to a sufficient depth over the pipe to prevent flotation or movement of the pipe in the trench.
Water from excavation shall be disposed of in such a manner as to not cause injury to public health, to public or private property, to the work completed or in progress, to the surface of the streets, or waterways or cause any interference with the use of the same by the public. When applicable, Contractor shall use erosion control measures specified by permits.
19. BACKFILL
Backfill material used by the Contractor shall be non-cohesive and non-plastic, free of all debris, lumps, clods, wood, broken paving or any organic or unstable material. Backfill material placed within one foot of the water lines shall not contain any stones or rocks larger than 2 inches in diameter and no stones or rocks larger than 6 inches in diameter will be permitted in any backfill.
Selected backfill material containing no stone or rocks larger than 2 inches shall be placed by the Contractor in 6-inch layers and thoroughly tamped to a depth of 12 inches over the top of the pipe. Particular attention and care shall be exercised by Contractor in obtaining thorough support for the branch of all service connection fittings. Care shall be taken to preserve the alignment and gradient of the installed pipe.
After selected backfill has been placed by the Contractor to a depth of 12 inches over the pipe, backfilling shall proceed to a depth of 30 inches over the pipe by placing the backfill material in 6-inch layers and thoroughly compacting with mechanical vibrations. Backfill in this portion of the work shall be compacted to 100 percent of maximum density of the material as hereinafter defined.
After the backfill has been placed by the Contractor to a level 30 inches over…
