About this file

This is a Customer Reference Questionnaire attachment from the Illinois Department of Transportation (IDOT) for a Construction and Materials Management System (CMMS) Modernization RFP (BidBuy Bid # 27-494DOT-FINAD-B-53247). The document serves as a scored evaluation component of vendor proposals, requiring offerors to submit completed questionnaires from current or past customers who directly experienced the vendor's solution and services. Offerors must distribute this form to their selected customer references, who must complete and submit the questionnaire directly to Sarah Beeman at Sarah.Beeman@illinois.gov with the bid number in the subject line by the proposal submission deadline. IDOT will only review a maximum of three customer reference questionnaires per offeror, and any submissions received after the proposal deadline will not be accepted and result in zero points for that reference.

The questionnaire requires customer representatives to provide organizational information, contact details, and signed certification, then respond to nine open-ended questions regarding the software solution's nature, implementation timeline, production environment dates, business line, customer role, lessons learned, and overall experience. Additionally, customers must rate the offeror's performance across eleven scaled criteria (1-5 scale), including system satisfaction, business needs alignment, configuration ease, data conversion success, system integration capability, project team expertise, contract compliance, training quality, customer support, product innovation confidence, and overall experience. References may be contacted by IDOT to verify responses and confirm the offeror's ability to deliver the required solution and services for the CMMS modernization project.
