About this file
This is Bid Amendment 2 for Request for Quote B-53182, issued by the Illinois Department of Public Health (DPH). The amendment consists of a Questions and Answers document addressing vendor inquiries submitted during the RFQ process and provides clarification on specific requirements for the contract.
The amendment clarifies production requirements for birth and death certificates, specifically addressing vendor concerns regarding copyright authorization and design reproduction rights. The Illinois DPH confirmed that sample products provided are for reference only and that all production must comply with RFQ specifications. For the certificate backside, the selected printer must incorporate a full bleed print using micro-text reading "ILLINOISBIRTHCERTIFICATE" for birth certificates and "ILLINOISDEATHCERTIFICATE" for death certificates, with an embossed relief pattern of the State of Illinois Official Seal created by bending the micro-text lines. The backside printing must match the main document border color—blue for birth certificates and green for death certificates—and no separate copyright authorization from third-party vendors is required, as the specifications define the technical requirements that must be met.

Text version
This bid amendment will consist of a Questions and Answers document includes relevant vendor questions and Illinois Department of Public Health (DPH) responses from the Request for Quote (RFQ).
Question: Could you please advise whether your office can provide authorization or written permission that allows the selected printer to reproduce the existing backside design? If the copyright is owned by a third-party vendor, could you also provide the necessary authorization or direct us to the appropriate rights holder?
Answer: The samples provided are simply samples of the current product for reference. Any production of the certificates must meet the requirements in the RFQ. Regarding the backside of the certificates, #13 on the RFQ must be met.
