Background Check Certification Form Summary

This is a Background Check Certification Form issued by the City of Palo Alto, California, required for organizations that provide classes, camps, or other services involving interaction with or supervision of minors under age 18 or vulnerable populations. The form requires organizations to certify that all employees, instructors, and service providers age 18 or older have passed a Livescan background check at the minimum service level of the California Department of Justice prior to providing services to the City. Additionally, organizations must certify that personnel have not been convicted of serious crimes or violent felonies as defined by California Education Code Section 45122.1 and Penal Code Sections 667.5 and 1192.7, and must adhere to California AB 506 requirements including mandated reporter training, child abuse prevention policies, and background check requirements for administrators, employees, and regular volunteers with direct contact with minors.

Organizations must submit a roster of individuals who have completed LiveScan clearance to risk@paloalto.gov, containing only the individual's full name and clearance date. Organizations must promptly notify the City if any certification information changes or if an individual becomes ineligible. The certification includes an indemnification clause requiring organizations to defend and hold harmless the City of Palo Alto and its officials from any claims arising from the organization's failure to conduct required background checks, failure to comply with applicable law, misrepresentation of eligibility, or assignment of ineligible individuals. An authorized representative must sign and date the certification form.
