BLK 4: CDRL tailored to include changes designated by AFRL/RVSV Program Manager (PM).
Contractor format, upon final approval of RVSV Program Manager, acceptable. The most current version of the Data Item Description is to be used.
Include all Government Furnished Equipment and Facilities required and the date and duration they will be required. Include a cost chart depicting by month the estimated cost, including subcontractor costs, for each major task.

Final program plans shall be marked with the appropriate distribution statement and any restrictive legends that apply. If restrictive legends are not evident, the data may be treated as unrestricted BLKS 10, 12, 13: The contractor shall deliver a draft Program Plan within 30 calendar days after award. Upon receipt of the comments, the contractor shall take no more than 15 days to incorporate all required changes and deliver the final version of the Program Plan. The plan shall be updated, as required and approved by Program Manager from the date of initial acceptance through technical completion.
BLK 14: An electronic copy of the program plan shall be delivered. One CD-ROM (CD-R), properly labeled, is the acceptable media for submittal. The most current version of MS Word shall be used. E-Mail transmittal is not permitted unless encrypted or secure.

The DID is tailored as follows:
1) Refer to DD Form 1586, Sample Format 1, CFSR Block:
11-14. Break out line items at CLIN level.
11f. Explain in Remarks, Block 15.
12-14. Include Forecasts by month for next 12 months, by remainder of GFY, then by year for remaining GFYs.
2) CFSR data shall be reconciled to the government’s fiscal year end of 30 September Electronic delivery via contractor-developed and operated secure communication application.

16. REMARKS
THE REPORT WILL BE PUBLISHED WITH THE DEFENSE TECHNICAL INFORMATION CENTER (DTIC) BY AFRL STINFO OFFICE. Under no circumstances shall the contractor submit either the final report or the camera-ready technical report to DTIC.
Blk 4: GUIDANCE FOR PREPARATION OF THE REPORT: The Scientific and Technical Report shall be prepared I.A.W. the most current AFRL ANSI/NISO Standard Z39.18-2005, or other most current report preparation guidance, located at: http://www.kirtland.af.mil/prs The contractor shall complete and submit an SF 298 Report Documentation Page, with the final report. The SF 298 is available at: http://www.kirtland.af.mil/shared/media/document/AFD-070425-037.doc. The contractor shall complete the following blocks of the SF298: Blocks 2-5a, 6, 7, 8 and 13 (if applicable), 14-16c.
The contractor shall complete and sign the most current AFRL Form 25, AFRL Quality Final Technical Report Checklist, and submit it with the final report as part of the report deliverable. The checklist is available at: http://www.kirtland.af.mil/prs. If the completed and signed AFRL Form 25 does not accompany the final report, the deliverable is considered incomplete. Incomplete final reports will be immediately returned to the contractor. Following AFRL technical and STINFO reviews, any revisions required will be returned to the contractor.
The contractor shall deliver two editable electronic copies of the final report. The electronic copies shall be an editable .pdf file and the most current version of Microsoft Word. A CD-ROM (CD-R) is the acceptable media for submittal. (CD-RWs are not acceptable.) Electronic media shall be marked IAW the AFRL ANSI/NISO Standard Z39.18-2005 guidelines. Submission via unencrypted or unsecure email is not permitted.
Blk 7: Submit Final Report with letter of transmittal and the DD 250 to the designated project manager. In preparing the final DD 250 for the contract, code the shipment number with a “Z”.
Blk 9: All technical data (including electronic media/CD-R) shall be marked I.A.W. DoDD 5230.24 with Distribution Statement __, and other required markings shown indented below:
Distribution authorized to [WUM fill in statement prior to award]; [WUM fill in reason(s); one of which must be Export Controlled if the report requires the Export Control Warning and none of which may be Proprietary Information], [contractor fill in date of report (dd mmm yy)]. Blks 10 - 13: The contractor shall deliver the Scientific and Technical Report, in final format, no later than the date specified in the Schedule, «CLIN 0002». The USAF will review and, if necessary, return the final report with comments no later than the date specified in the Schedule, «CLIN 0002». If the report is returned to the contractor with comments, the contractor shall deliver the camera-ready copy of the report no later than the date specified in the Schedule, «CLIN 0002».
NOTE: Headings in Block 14B, Copies, have been changed. “Draft” is changed to “Final” and “Final” (in second column) is changed to “CRC” (camera-ready copy).

During performance of this contract the contractor may purchase material or equipment using government funds (Contractor Acquired Property (CAP)) as specified in the contract. The contractor shall annually perform, record, and disclose physical inventory results of all CAP material/equipment. Ninety (90) days prior to contract expiration the contractor shall submit a final CAP Property List suitable for close-out purposes containing use/disposition recommendations and approved by the Contract Program Manager.
The CAP Property List shall include:
- List of CAP by nomenclature
- National Stock Number of each equipment
- Serial Number of each equipment
- Quantity of each equipment
- Condition of each equipment, in accordance with the equipment condition status listed:
-- Service Usable Without Qualification. New, Used, Repaired, or Reconditioned.
-- Service Usable With Qualification. New, Used, Repaired or Reconditioned equipment which is serviceable and useable for its intended purposes but which is restricted from issue to specific units, activities, or geographical areas by reason of its limited usefulness or short service-life expectancy.
-- Serviceable Test/Modification. Serviceable equipment which requires test, alteration, modification, conversion or disassembly.
- Status of the CAP.
- Solutions of problems relating to the above.
- Request for assistance for repair of CAP, as necessary.
The CAP Property List shall be delivered in a spreadsheet using Microsoft Office Excel. Abbreviations are not allowed.
