WILLOW VALLEY CREEK BRIDGE

August 2016

1.3 Nature and Sequence of Construction Activity

The Federal Highway Administration, Central Federal Lands Highway Division (FHWA- CFLHD), in cooperation with the Coconino National Forest (CNF) and Coconino County, is proposing improvements to Lake Mary Road (Forest Highway 3 / County Road 209) to address safety concerns and general roadway deficiencies (drainage, curve radii, sight distance etc.).

Lake Mary Road is located approximately 40 miles to the southeast of Flagstaff, Coconino County, Arizona. This road provides access to private and U.S. Forest Service (USFS)-managed lands between Interstate 17 to the north and State Route 87 to the south. This route accesses Lake Mary, Mormon Lake, residential cabins and homes, and the recreational lands of the Coconino National Forest (CNF). The proposed project includes three priority areas. In summary, Priority Area 1 includes the rehabilitation of the Willow Valley Creek Bridge, located at the approximate milepost (MP) 297.5, and Priority Areas 2 and 3 include the roadway improvements of approximately 13.5 miles of roadway between MP 310.0 and 323.5.

Construction will begin in April 2017 and will be completed by October 2017. Construction will be completed in the following sequence:

1. Establish temporary Best Management Practices (BMPs)

2. Removal of obstructions

3. Topsoil management

4. Roadway excavation and ditch excavation

5. Culvert and headwall installation

6. Slope protection and sediment wattle installation

7. Roadway pulverizing and aggregate base finishing

8. Concrete Curb and ditch installation

9. Paving and asphalt curb installation

1.4 Soils, Slopes, Vegetation, and Current Drainage Patterns

Soil type(s):

The dominant soil orders in this MLRA are Borolls, Boralfs, Ustolls, Ustalfs, Ostalfs, Orthents, and Orthids. They have a cyric, frigid, or mesic temperature regime depending mainly on elevation. Argiborolls and Cryoborolls are formed at the higher elevations in intrusive and extrusive volcanic materials, dominantly basalt. At lower elevations, Eutroboralfs, Ustorthents, Haplustalfs, Argiustolls, Haplustolls, Calciorthids, and Glossoboralfs overlie on schist, sandstone, shale, limestone, and old alluvium.

According to the U.S. Department of Agriculture - Natural Resources Conservation Service’s (USDA-NRCS) online Web Soil Survey (http://websoilsurvey.nrcs.usda.gov/app/WebSoilSurvey.aspx), soils for the majority of the project area are not mapped. However, the portion of mapped soils are typical for the entire project area. Mapped soils within the project area are composed of Friana soils (Fn) (Fn), and Brolliar very stony loam, 0 to 10 percent slopes (BrB). The Friana soils are generally described as moderately well drained alluvium soils derived from basalt, cinders, and/or pyroclastic rock.

These soils have very low to moderately low permeability and high water capacity. The Brolliar very stony loam is generally described as well drained colluvium soils derived from basalt.

These soils have very low to low permeability and low water capacity.

Slopes:

Elevation varies from 7300 ft to 7500 ft above mean sea level along the project length on Lake Mary Road. Willow Creek Bridge is at an elevation of 6735ft above mean sea level. Slopes are along the project are gradual with roadside ditches present along part of the alignment.

Vegetation:

Vegetative cover within this region of the MLRA typically consists of alpine vegetation, conifer forests, chaparral, and grasses because of the broad elevation range. The understory includes Thurber fescue, brome, bluegrasses, mountain muhly, Arizona fescue, lupine, aspen peavine, penstemons, and daisies. The major part of the area is a vast ponderosa pine forest.

Current Drainage Patterns:

The project area lies within the Colorado and Mogollon Plateau region of Arizona. Well established drainage on the Colorado Plateau does not exist due to the nature of the rock formations. Therefore, precipitation sinks down through underground channels and caverns which again reach surface at lower elevations.

1.5 Construction Site Estimates

The following are estimates of the construction site.

Total project area: XXX acres

Construction site area to be disturbed : XXX acres

Percentage impervious area before construction: XX %

Runoff coefficient before construction: 0.XX

Percentage impervious area after construction: XX %

Runoff coefficient after construction 0.XX

This project is a rehabilitation of an existing roadway with only a portion of the roadway being widened. Therefore the runoff coefficient changed slightly.

3.1 Spill Prevention and Control Plan(ADEQ CGP 6.3.11)

The on-site construction contractor shall develop a plan describing what actions will be taken in case of a spill and incorporate preventative measures to be implemented (such as the placement of refueling facilities, storage and handling of materials, etc.). This plan must be made at least 2 days before beginning work and contain the following.

Spills of Petroleum Products or Hazardous Materials: Procedures to properly clean up, mitigate, and remedy, if necessary, all spills of petroleum products, hazardous materials, or other

WILLOW VALLEY CREEK BRIDGE

August 2016 chemical or biological products released from construction, fleet, or other support vehicles, or stationary sources. Respond in accordance with federal, state, and local regulations.

Immediately report to the CO any spill of petroleum products or a hazardous material. Report the spill to the appropriate federal, state, and local authorities as required by the SPCC plan or hazardous spill plan.

Contractor to insert additional information on spill plan.

Water pollution The on-site construction contractor shall not operate mechanized equipment or discharge or otherwise place any material within the wetted perimeter of any waters of the U.S.

within the scope of the Clean Water Act (33 USC § 1251 et seq.). This includes wetlands unless authorized by a permit issued by the U.S. Army Corps of Engineers according to 33 USC § 1344, and, if required, by any State agency having jurisdiction over the discharge of material into the waters of the U.S. In the event of an unauthorized discharge:

(1) Immediately prevent further contamination;

(2) Immediately notify appropriate authorities; and

(3) Mitigate damages as required.

Comply with the terms and conditions of any permits that are issued for the performance of work within the wetted perimeter of the waters of the U.S.

Separate work areas, including material sources, shall use a dike or other suitable barrier to prevent sediment, petroleum products, chemicals, or other liquid or solid material from entering the waters of the U.S. Use care in constructing and removing the barriers to avoid any discharge of material into, or the siltation of, the water. Remove and properly dispose of the sediment or other material collected by the barrier.

Vehicles and equipment All vehicles and equipment entering the project area must be clean of noxious weeds and free from oil leaks and are subject to inspection. Wash all construction equipment to thoroughly remove all dirt, plant, and other foreign material prior to entering the project. Particular attention must be shown to the under carriage and any surface where soil containing exotic seeds may exist. These efforts are critical to prevent the introduction and establishment of non-native plant species into the project area. Make arrangements for the CO to inspect each piece of equipment before entering the project. The CO will maintain records of inspections. Equipment found operating on the project that has not been inspected, or has oil leaks will be shut down and subject to citation.

In general, when gasoline, diesel fuel, antifreeze, hydraulic fluid or any other chemical contained within the vehicle is released to the pavement or ground, proper corrective, clean-up, and safety actions must be immediately implemented. All vehicles with load rating of 2 tons or greater should carry, at minimum, enough absorbent materials to effectively immobilize the total volume of fluids contained within the vehicle.

WILLOW VALLEY CREEK BRIDGE

August 2016

Repair oil leaks immediately on discovery. Do not use equipment that is leaking. Have oil pans and absorbent material in place prior to beginning repair work. Have the “on scene” capability of catching and absorbing leaks or spillages of petroleum products including antifreeze from breakdowns or repair actions with approved absorbent materials. Keep a supply of acceptable absorbent materials at the job site in the event of spills. Sand and/or soil are not approved absorbent materials.

Use oil pans and absorbent materials to prevent leaks, spills and draining petroleum fluids from falling onto bare ground and paved surfaces during servicing of equipment. Dig up soils contaminated with such fluids, place in appropriate safety containers, and dispose of according to state and/or federal regulations.

Clearances for Contractor-selected, noncommercial areas Noncommercial areas include material sources, disposal sites, waste areas, haul roads, and staging areas located outside project construction limits and permitted commercial areas. Permitted commercial areas are enterprises or developed areas providing same-type material or use over the last 2 years with appropriate permits.

Before using a Contractor-selected, noncommercial areas, submit the following:

Locate machinery servicing and refueling areas away from streambeds and washes to reduce the possibility and minimize the impacts of accidental spills or discharges.

Where the Contractor’s working area encroaches on a running or intermittent stream, construct and maintain adequate barriers to prevent the discharge of any contaminants into the stream.

Fording of running streams with construction equipment will not be permitted. Obtain approval from the CO to use temporary bridges or other structures whenever crossings are necessary.

Immediately clear ephemeral drainages, intermittent and perennial streams, lakes and reservoirs of all work items, debris or other obstructions resulting from construction operations.

WILLOW VALLEY CREEK BRIDGE

August 2016

Spill Prevention and Control Procedures BMP Description: A spill kit (i.e. fueling spill control blanket) will be provided for use during mobile refueling. Staff will be trained in the proper usage of the spill kit. Contractor will follow comprehensive handling and management procedures to prevent hazardous liquids from migrating across the site and to avoid groundwater contamination. Containment measures (e.g., drip pans, drip cloths, or absorbent pads) will be used when replacing spent fluids.

Contractor will train employees in proper equipment handling and refueling procedures.

Installation Schedule: Refueling and maintenance procedures to be established prior to construction.

Maintenance and Inspection:

Areas where refueling/maintenance occur will be inspected daily for signs of storm water pollutant exposure. Inspect equipment for leaks of oil, fuels, or hydraulic fluids before and during use. Maintenance and inspections will be documented in corrective action will be documented in Appendix E (Corrective Action Log) and Appendix G (Inspection Reports) of the

SWPPP.

Responsible Staff: Field Contractor

3.2 Allowable Non-Stormwater Discharge Management (ADEQ CGP

1.3.2)

The following are the only non-stormwater discharges allowed under ADEQ CGP. These discharges are allowed provided they are reduced or eliminated to the extent practicable. When allowable non-stormwater discharges cannot be practicably eliminated, the operator shall install appropriate control measures to reduce or eliminate pollutants in the discharge to assure compliance with Part 3 of the ADEQ CGP.

i. Discharges from emergency fire-fighting activities;

ii. Water used to control dust, provided reclaimed water or other process wastewaters are not used;

iii. Routine external building wash down where detergents are not used;

iv. Water used to rinse vehicles and equipment, provided that reclaimed water or other wastewater is not used and no soaps, solvents, detergents, oils, grease or fuels are present in the rinsate;

v. Pavement wash waters where spills or leaks of toxic or hazardous materials have not occurred (unless all spilled material has been removed) and where detergents are not used;

vi. Uncontaminated air conditioning or compressor condensate;

vii. Uncontaminated groundwater or spring water;

viii. Foundation or footing drains where flows are not contaminated with process materials such as solvents;

WILLOW VALLEY CREEK BRIDGE

August 2016

ix. Water from fire fighting system testing and maintenance, including hydrant flushings;

x. Discharges related to installation and maintenance of potable water supply systems, including disinfection and flushing activities, discharges resulting from pressure releases or overflows, and discharges from wells approved by ADEQ for drinking water use;

xi. Hydrostatic testing of new pipes, tanks or vessels using potable water, surface water, or uncontaminated groundwater;

xii. Water used for compacting soil, provided reclaimed water or other wastewaters are not used;

xiii. Water used for drilling and coring such as for evaluation of foundation materials, where flows are not contaminated with additives; and

xiv. Uncontaminated waters obtained from dewatering operations/ foundations in preparation for and during excavation and construction provided the discharge are managed as specified in Part 3.1.4 of the ADEQ CGP.

Note: The ADEQ CGP does not prohibit the use of reclaimed or other process wastewaters on-site for dust control, soil compaction or for landscape irrigation. However, such activities shall be managed in a way that they are not discharged off site or applied during rain events.

Any changes in construction activities that produce other allowable non-stormwater discharges will be identified, the SWPPP will be amended, and the appropriate erosion and sediment controls will be implemented.

Water Used to Control Dust

BMP Description: Dust control will be implemented, as needed, once site grading has been initiated and during windy conditions while site grading is occurring. Dust control will continue on-site during construction activities, as needed.

Responsible Staff: Field Contractor

Uncontaminated Excavation Dewatering

BMP Description: Because construction for this site is being conducted during the dry season, dewatering activities are not expected to occur at the project site. If dewatering does occur, the SWPPP will be revised to address the need for appropriate BMPs.

Responsible Staff: Field Contractor

WILLOW VALLEY CREEK BRIDGE

WILLOW VALLEY CREEK BRIDGE

August 2016

SECTION 4: Erosion and Sediment Control Contractor shall not disturb existing vegetation outside project limits and shall minimize the impacts to existing vegetation and side slopes within construction limits. Temporary and/or permanent seeding within the construction right of way shall be accomplished using a seed mix determined by the Coconino Forest Service. Supplemental seeding may be performed by the Forest Service upon completion of construction. Existing topsoil not be removed from site.

Specific locations for salvaging topsoil shall be identified during construction phases and located on the site plan. Only certified weed free erosion control devices will be used.

Stormwater flow shall be controlled by the existing, as the project progresses, replaced culverts.

Erosion control will consist of implementing the use of perimeter controls and sediment barriers including silt fence and check dams as shown on the plan set. During construction, temporary inlet and outlet protection shall as needed within the area of disturbance. Wind erosion control shall consist of application of water and/or application of city, state and federally approved chemical dust suppressants to exposed surfaces that cause a dust nuisance. Construction entrances will be stabilized (i.e. track out protection and wash down) through the duration of the project. The contractor shall minimize the amount of soil exposed during construction activities and subject to the deadlines for temporarily and/or permanently stabilizing portions of the site.

Initiate work to fix problem immediately after discovery, and complete such work by the close of the next work day, if feasible and the problem does not require significant maintenance, repair or replacement.

The contractor shall complete the installation of control measures by the time each phase of earth-disturbance has begun. Where practicable, implement standard erosion and sediment control practices to these areas and using stabilization practices designed to be used on steep grades. In the event it is infeasible to install one or more control measures prior to construction activity, the operator shall ensure that those controls are installed as soon as possible. SWPPP records must document why it is infeasible.

The contractor shall control discharges from stockpiles of sediment or soil. Stockpiles shall be place outside of washes or other surface waters, or stormwater conveyances, such as curb and gutter systems, or streets leading to such conveyances. Stockpiles shall be protect from contact with stormwater (including run-on) using a temporary perimeter sediment barrier. Silt fences or other effective sediment control measures shall be used around soil stockpiles except when they are being actively worked.

The contractor shall protect culverts present on the site by include measures to sufficiently minimize the threat of erosion at culvert locations and prevent the formation of rills and gullies during construction. The contractor shall place velocity dissipation devices along the length of any on-site outfall channel where discharges leaving the construction site cause rill erosion to maintain a non-erosive flow velocity.

Note: Inlet protection measures can be removed in the event of flood conditions that may endanger the safety of the public. Such actions are allowable only under extreme

WILLOW VALLEY CREEK BRIDGE

August 2016 conditions and shall be documented on the SWPPP. The contractor shall evaluate alternatives to be used in the future to prevent a recurrence of this problem.

If existing control measures need to be repaired or modified or if additional control measures are necessary, implementation shall be completed within 7 calendar days or before the next storm event (whichever is sooner). If implementation before the next storm event is impracticable, the reason(s) for delay shall be documented in the SWPPP and alternative control measures shall be implemented as soon as possible. Accumulations of sediment, debris, and other pollutants observed in off-site surface waters, drainage ways, catch basins, and other drainage features shall be removed in a manner and at a frequency sufficient to minimize impacts and to ensure no adverse effects on water quality. The following maintenance activities shall be implemented as follows:

Remove accumulated sediment when it reaches a maximum of one-third the height of the silt fence or one-half the height of a fiber roll.

Sediment shall be removed from temporary and permanent sedimentation basins, ponds and traps when the depth of sediment collected in the basin reaches 50% of the storage capacity.

Construction site egress location(s) shall be inspected for evidence of off-site tracking of sediment, debris, and other pollutants onto paved surfaces. Removal of sediment, debris, and other pollutants from all off-site paved areas shall be completed as soon as practicable.

4.1 Phase Construction Activity

The Federal Highway Administration (FHWA), Central Federal Lands Highway Division (CFLHD), in cooperation with the U.S. Forest Service (USFS), is proposing to rehabilitate, reconstruct, and resurface Lake Mary Road and the Willow Valley Creek Bridge in Coconino National Forest, Coconino County, Arizona. The proposed project would include roadway improvements of approximately 13.5 miles of roadway between milepost (MP) 310.0 and 323.5 as well as the bridge located approximately at MP 297.5. The project would involve roadway geometry improvements, roadway pavement improvements, roadway safety improvements, culvert replacements, and bridge replacement.

Construction will begin in April 2017 and will be completed by October 2017. Construction will be completed in the following sequence:

1. Contractor to outline sequence of activities

4.2 Structural Practices for Erosion and Sediment Control

The following list applicable BMPs that may be used for this project. Several of these BMPs have been called out on the site plan in Appendix A. When additional BMPs are added or modified, the site plan shall be updated by indicating the location of BMP, signed and date as to when it was installed and/or removed by marking up the plan set by hand. If the site plan

WILLOW VALLEY CREEK BRIDGE

August 2016 becomes hard to read, additional phases of the plan set can be attached to indicate the different phases of the plan.

All temporary BMPs shall be removed and stabilized prior to submitting the Notice of Termination with ADEQ.

4.2.1 Natural Buffers or Equivalent Sediment Controls (CPG-2013 3.1.1.5, 6.3(5)e, 6.4(10))

Buffer Compliance Alternatives Are there any surface waters within 50 feet of your project’s earth disturbances? YES NO

Check the compliance alternative that you have chosen:

I will provide and maintain a 50-foot undisturbed natural buffer.

I will provide and maintain an undisturbed natural buffer that is less than 50 feet and is supplemented by additional erosion and sediment controls, which in combination achieves the sediment load reduction equivalent to a 50-foot undisturbed natural buffer.

It is infeasible to provide and maintain an undisturbed natural buffer of any size, therefore I will implement erosion and sediment controls that achieve the sediment load reduction equivalent to a 50-foot undisturbed natural buffer.

I qualify for one of the exceptions in Part 2.1.2.1.e. (If you have checked this box, provide information on the applicable buffer exception that applies, below.)

Buffer Exceptions Which of the following exceptions to the buffer requirements applies to your site?

There is no discharge of stormwater to the surface water that is located 50 feet from my construction disturbances.

No natural buffer exists due to preexisting development disturbances that occurred prior to the initiation of planning for this project.

No buffer exists due to previous road construction disturbance For a “linear project” (defined in Appendix A), site constraints (e.g., limited right-of-way) make it infeasible for me to meet any of the CGP Part 2.1.2.1.a compliance alternatives.

All areas within 50 feet will have at least one sediment control device between the construction and the surface water if stormwater can flow from the construction area into a jurisdictional water.

The project qualifies as “small residential lot” construction (defined in Part 2.1.2.1.e.iv and in Appendix A).

Buffer disturbances are authorized under a CWA Section 404 permit.

This project meets the requirements of Section 404 NWP-14 (Linear Transportation). A copy of the 404 permit requirements is included in the appendices.

Buffer disturbances will occur for the construction of a water-dependent structure or water access area (e.g., pier, boat ramp, and trail).

WILLOW VALLEY CREEK BRIDGE

August 2016

4.2.2 Perimeter Controls

General

Perimeter controls will be placed prior to earth disturbance to prevent sediment from leaving the job site. Additional silt fence may be installed down gradient of a disturbance where sediment is anticipated to leave the job site.

Specific Perimeter Controls (ADEQ CGP 3.1.1.4.1)

Perimeter Control # 1 Silt Fence

Silt fence will be installed prior to beginning earth disturbances along the project on the down gradient side where water leaves the project.

Specifications and standard detail drawings are included in the contract documents.

Installation Perimeter controls will be installed prior to earth disturbances on the project.

Maintenance Requirements Sediment will be removed before it has accumulated to one-half of the above-ground height of any perimeter control.

4.2.3 Sediment Track-Out (ADEQ CGP 3.1.3.2, 6.3(8)d)

General

Where sediment has been tracked-out from the site onto the surface of off-site streets, other paved areas, and sidewalks, remove the deposited sediment by the end of the same work day in which the track-out occurs or by the end of the next work day if track-out occurs on a non-work day. Remove the track-out by sweeping, shoveling, or vacuuming these surfaces, or by using other similarly effective means of sediment removal. You are prohibited from hosing or sweeping tracked-out sediment into any stormwater conveyance (unless it is connected to a sediment basin, sediment trap, or similarly effective control), storm drain inlet, or surface water.

4.2.4 Stockpiled Sediment or Soil (ADEQ CGP 3.1.1.4)

General

Stockpiles of material will be limited at the site, but may occur occasionally while specific operations are taking place. Potential earthen materials that may be stockpiled for short periods of time include riprap and structural backfill materials. Sediment controls will be placed around stockpiles that may release sediment in a storm event.

The type of sediment control device used will depend on the location of the stockpile (on paved or unpaved surfaces). The contractor may choose the type of control that best fits the conditions of the project.

Specific Stockpile Controls

4.4 Good Housekeeping BMPs (ADEQ CGP 3.1.3.3)

Good housekeeping shall be used to keep potential areas where pollutants exist clean and orderly. Containers, drums, and bags shall be stored away from direct traffic routes to reduce the risk of accidental spills. Stack containers according to manufacturer’s instructions to avoid damaging the containers from improper weight distribution. Containers will be stored on pallets or similar devices to prevent corrosion of containers that results from containers coming in contact with moisture on the ground. Hazardous liquids shall be stored within curbed areas or secondary containment. Secondary containment will be placed around (i.e. sediment control logs) or under (i.e. tarps) semi-stationary portable equipment such as generators and compressors, as this equipment is generally maintained and re-fueled while on-site.

At the end of a shift and before an oncoming storm event, the site inspector shall ensure that all trash bins are secured, and spills have been cleaned up.

Power wash all vehicles and equipment offsite, prior to entering the jobsite to remove dirt, mud, plant materials, non-native seed and noxious weed seed-bearing material. Hauling vehicles shall meet the same requirement before their initial entrance into the park; subsequent entries will not require cleaning unless ordered by the Contracting Officer.

Waste Materials

BMP Description: All waste materials will be collected and disposed of daily into trash dumpsters located in the materials storage area. Dumpsters will have a secure watertight lid, be placed away from storm water conveyances and drains, and meet all federal, state, and municipal regulations. Only trash and construction debris from the site will be deposited in the dumpster.

No construction materials will be buried on-site. All personnel will be instructed, during tailgate training sessions, regarding the correct disposal of trash and construction debris. Notices that state these practices will be posted in the office trailer and the individual who manages day-to-day site operations will be responsible for seeing that these practices are followed.

Installation Schedule: Trash dumpsters will be installed once the materials storage area has been established.

Maintenance and Inspection:

The dumpsters will be inspected weekly and immediately after storm events and will be emptied at least weekly and taken off-site for disposal.
