CONSTRUCTION NOTES

BRIDGE OVERLOAD PERMITS

All loads crossing bridges under Forest Service jurisdiction, and over 40 tons transported to the site will require an overload permit. Each load will require a permit for each proposed bridge crossing. It is the Contractor's responsibility to submit permit application information to the Forest Service including transport vehicle, load configuration, and specific axle measurements (including weight). The overload review and permit process takes approximately 2 weeks, and the Contractor should plan accordingly. No overloads will be allowed to cross specific bridges without an approved and signed permit in possession of the driver.

WATER FOR PROJECT USE

Water is incidental to all items in this contract.

Work consists of developing an acceptable water supply, furnishing, hauling, and applying water. Use designated water sources, or other approved water sources with appropriate permissions, water rights, and permits. Provide mobile watering and pumping equipment with water tight tanks of known capacity. Apply water uniformly without ponding or washing.

Water Source Development Requirements Withdrawal shall not exceed 1/4 of the flow between April 15 - July 15. After July 15, retention of 50% of base flow should be sufficient. No withdrawals will be permitted from any stream when the flow falls below 1 CFS (approximately 8 gal/ sec.). Pumping or damming activities which dewater a stream will not be allowed. Pumping equipment is necessary at specified water sources. Screen water pump intakes and draft hoses used to withdraw water from ponds or streams shall be screened with appropriate size mesh to prevent entrapping fish. Screening equipment shall meet the following minimum dimensions:

- Woven wire screen or perforated plate: 3/32"

- Profile bar screen: 0.0689"

ITEM 15207 GRADE FINISH STAKES, TOLERANCE CLASS B

Work includes reference staking existing road dimensions (horizontal location, cross slope, and profile) prior to commencement of reclamation or excavation activities. Conventional staking methods and electronic reference methods are allowed. It is the Contractor's responsibility to propose a method accurate enough to reconstruct the roadway to dimensions specified in these contract drawings with the road centerline, grade, and superelevation cross slopes matching pre-project location and dimsnesions.

Commencement of reclamation or excavation shall not occur until the Forest Service approves a staking plan proposed by the contractor.

ITEM 20429 WASTE

Work includes haul and placement in disposal site of unsuitable material from roadway repair (20409), ditch reconditioning (30301) and excess material generated during cattle guard removal, replacement and reinstall.

ITEM 30311 AGGREGATE SURFACE RECONDITIONING, COMPACTION METHOD A, (DETOUR MAINTENANCE)

Work includes surface maintenance of the designated detour route for time periods that portions of the 42 road are closed. At minimum, the Contractor shall blade the entire detour route at least once per operating season (once per year during the project). Additional maintenance may be required, and is the responsibility of the contractor, to maintain suitable conditions for public passenger vehicles. Water is incidental to this item. Use sources designated in contract documents and alternate sources identified in writing by the Forest Service.

ITEM 41801B MINOR SURFACE ASPHALT PATCHING

Work includes minor asphalt patching of pot holes and other small road surface imperfections. Locations are sporadically located throughout from the junction of forest road 22 to the project area, on forest road 42, as staked by the forest service.

Patch dimensions and depths vary, and alternate spreading methods will be required to accomplish the work. Apply tack oil as specified to entire patch area. Specific locations and dimensions shall be marked on the ground prior to the commencement of work and throughout the duration of the project.

ITEM 63402 PAVEMENT MARKINGS, TYPE B

Work includes centerline striping on road 42 from road 22 junction to 30 road junction. It is the contractor's responsibility to re-stripe with the same road marking pattern and in the same location as existing centerline stripes. The contractor shall stake, or use other means to reference existing pattern prior to activities that will remove existing striping (such as full depth reclamation or skin patching). The Forest Service will identify the desired striping pattern in sections that existing striping is not present.

ITEM 63501 TEMPORARY TRAFFIC CONTROL

All temporary traffic control measures shall be installed and maintained according to the most current mutcd edition, fp-14 section 653, and all other requirements in this contract. At minimum, install traffic control signs listed on sheet 5 of these drawings. Temporary traffic control devices shall not be installed on existing sign posts, and to the extent possible shall not obstruct visibility of existing signs. Additional traffic control measures may be necessary, and are the responsibility of the contractor. Deviations from the required minimum traffic control plan shall be proposed by the contractor and approved in writing by the forest service prior to implementation. Portions of road 42 provide access to private property. When reasonably possible, and safe to do so; the Contractor shall work with private property owners to allow access to their property. The Forest Service will communicate estimated schedules and changes in schedule to affected private property owners, and exchange direct contact information between the contractor and landowners to facilitate access.
