About this file
Attachment I is a Foreign Country of Concern Attestation form (PUR 1355) issued by the Florida Department of Management Services for the Management Consulting Services RFP No. 25-80101500-RFP-V2. This specific document is a mandatory certification form that must be completed by an officer or representative of any entity submitting a bid, proposal, or contract to a governmental entity where the entity would have access to personal identifying information. The form requires the bidding entity to attest that they are not owned by, organized under the laws of, or have a principal place of business in a "Foreign Country of Concern," and that no such government has a controlling interest in the entity.
The attestation form is part of a broader procurement process for a statewide management consulting services contract, with an anticipated annual spend of approximately $90 million. Vendors must complete this form as part of their mandatory documentation, along with other certifications like the Vendor Certification Form and No Offshoring declaration. The procurement covers nine service categories including organizational strategy, project management, policy consulting, and grants management, with consultants to be evaluated on a 100-point system (90 points for technical proposal, 10 points for cost). Key procurement dates include a solicitation posting on April 9, 2025, with proposal submissions due by June 17, 2025, and the Department reserving the right to make multiple statewide awards or no award at all.
