About this file
This is a Customer Reference Questionnaire attachment for the Illinois Department of Transportation's Aeronautics Program Management System (APMS) RFP 26-494DOT-FINAD-B-50780. The questionnaire serves as a scored component of proposals submitted in response to the RFP, requiring offerors to obtain completed reference forms from current and/or past customers who directly experienced the vendor's solution and services. Customer Reference Questionnaires must be submitted directly to Sarah.Beeman@illinois.gov with the bid number in the subject line by the proposal submission deadline; questionnaires received after this deadline will result in zero points awarded for that reference. IDOT will review no more than three completed customer reference questionnaires per offeror, and references may be contacted by IDOT to verify customer responses and the offeror's capability to provide the solution or services.
The Customer Reference Questionnaire requires detailed information about the customer organization and the representative providing the reference, including organization name, address, representative title, phone number, and email. Respondents must address nine open-ended questions covering the type of aeronautics program management software solution provided, implementation timeline, production environment dates, primary line of business, the respondent's specific role, consideration of competing systems, lessons learned, and additional relevant experience. Additionally, respondents must rate eleven specific evaluation criteria on a five-point scale addressing system satisfaction, business needs alignment, configuration ease, data conversion success, integration success, project team skill and experience, implementation per contract terms, training services, customer support quality, confidence in the vendor's innovation roadmap, and overall experience with the offeror.
