About this file
This is a Small Business Subcontracting Plan (Attachment D) for IFB #123913 issued by the Commonwealth of Virginia for the procurement of Microscopes and Accessories. The plan applies to all potential offerors responding to this invitation for bids and establishes requirements for small business participation in the contract performance. Offerors must include the completed Small Business Subcontracting Plan with their proposal response. The Commonwealth has established a goal that over 42% of its purchases be made from businesses certified by the Virginia Department of Small Business and Supplier Diversity (DSBSD), including small businesses, micro businesses, women-owned businesses, minority-owned businesses, and service-disabled veteran-owned businesses.
Offerors certified by DSBSD as small or micro businesses need only complete Section A of the form by providing their certification number and certification date to receive maximum available points for the small business participation evaluation criterion. Offerors not certified as small businesses must complete Section B, which requires identification of DSBSD-certified small business subcontractors and the specific dollar amounts or percentages of contract value to be subcontracted to such firms for the initial contract period. The form accommodates up to five subcontract entries, each requiring company name, DSBSD certification number, contact information, certification type, work description, and proposed dollar value or percentage of the initial term. Proposed small business participation plans will be incorporated into the contract as binding requirements, and failure to achieve proposed participation levels may result in breach of contract.
