About this file

This is a Confidentiality Agreement (Form SCVR 457, Revised 12/2018) issued by the South Carolina Vocational Rehabilitation Department (SCVRD) that applies to all employees, interns, practicum students, work-study students, and contractors working with or conducting business with the agency. The agreement establishes mandatory confidentiality protocols for handling restricted and confidential information, including federal, state, and local policy-protected materials. Covered information includes medical and psychological records, personally identifiable information (PII), social security numbers, payroll documents, personnel files, home addresses, telephone numbers, and consumer files in both physical and electronic formats. All individuals must secure restricted materials when not in use, prohibit unauthorized access, refrain from making personal copies without authorization, and assume all information is confidential unless otherwise indicated. The agreement explicitly prohibits careless discussion of consumer or employee information, unauthorized access to restricted data, sharing information outside proper authorization channels, breaching confidentiality through social media, and improper disposal of confidential documents.

The agreement requires all personnel to report confidentiality breaches to a SCVRD supervisor, legal department, information technology department, or human resources department. Violations of the confidentiality standards are subject to disciplinary action and may include unauthorized disclosure, falsification, or destruction of records. Signed acknowledgment of the agreement is maintained in each employee's personnel file, and the standards apply both during employment and after employment, volunteering, or business with SCVRD has ended. The agreement establishes that maintaining strict confidentiality of all accessible materials, processes, and information is a personal responsibility for all parties subject to this policy.
