About this file

This is a work plan detail attachment for RFA #20770 issued by the New York State Department of Health, Office of Health Equity and Human Rights, establishing performance objectives and measurable outcomes for Component 2 of the Migrant and Seasonal Worker Program. The contract targets services to children of migrant and seasonal workers across New York State, with a four-year contract period valued at $1,405,724 ($351,431 annually). The work plan comprises six primary objectives: Outreach (to increase access to health and human services), Engagement (to identify service needs and barriers), Linguistically Appropriate and Culturally Sensitive Services (including translation and interpretation), Health Screenings (covering hearing, vision, blood pressure, infectious disease testing, and mental health screening), Comprehensive Health Care (preventive, primary, dental, specialty, mental health, and emergency services), and Human Service Needs (insurance enrollment, entitlements, food, housing, and education services).

Performance measures are established for each objective to track outcomes including the number of children reached through outreach, attendance at outreach events, health education participation, language interpretation and translation services provided, health screening completion rates, follow-up care for positive screens, preventive and specialty care visits, and enrollment in health insurance and benefits programs. The work plan requires service providers to develop and implement detailed plans for outreach, engagement, and language access, provide narratives demonstrating cultural responsiveness and system implementation, and document referrals and direct service provision across all program components. The contract is funded through New York State and federal sources, targeting qualified public and not-for-profit community-based health and human service organizations with recognized ties to the migrant and seasonal worker population.
