About this file
This work plan summary document outlines the performance objectives and deliverables for Component 1 of RFA #20770, issued by the New York State Department of Health, Office of Health Equity and Human Rights, to provide services to Migrant and Seasonal Workers (MSW) and their families across designated economic development regions in New York State. The contract period spans four years with annual funding of $351,431, totaling $1,405,724, and requires selected organizations to implement six primary workplan objectives: (1) Outreach to increase access to health and human services; (2) Engagement to identify service needs and barriers; (3) Provision of linguistically appropriate and culturally sensitive services; (4) Comprehensive health screenings including blood pressure, diabetes, STI/HIV, hepatitis, tuberculosis, mental health, and domestic violence assessments; (5) Comprehensive health care services encompassing preventive, primary, dental, specialty, mental health, substance use, and emergency care; and (6) Human service needs addressing health insurance enrollment, entitlements, benefits, and other essential services including food, housing, and education.
Each objective contains specific tasks and associated performance measures requiring documented reporting on client reach, service utilization, and outcomes. Contractors must track quantitative metrics such as the number of families reached through outreach, health education participants, individuals receiving language interpretation and translation services, clients receiving health screenings and subsequent counseling, primary and preventive care visits, and referrals or direct services for human service needs. Qualitative reporting requirements include narratives demonstrating accessible service delivery, culturally responsive health services, language access plan effectiveness, and follow-up systems for positive health screenings. The work plan emphasizes direct engagement with the MSW population to ensure services are tailored to their specific needs and barriers while maintaining comprehensive coverage across preventive, urgent, and specialty care domains.
