About this file
This is a Worker Protection and Investment Certification Form issued by the Pennsylvania Department of General Service, Public Works, applicable to contractors bidding on state contracts, including the PLCB Security Guard Services opportunity. Contractors must certify compliance with thirteen categories of Pennsylvania state labor and workforce safety laws, including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, Employer to Pay Employment Medical Examination Fee Act, Seasonal Farm Labor Act, Wage Payment and Collection Law, Industrial Homework Law, Construction Industry Employee Verification Act, Act 102 regarding excessive overtime in healthcare, Apprenticeship and Training Act, and the Inspection of Employment Records Law. The certification requires an authorized company official to execute the form, confirming the firm's compliance with all listed labor and safety laws and understanding of their obligation to report any changes in compliance status to DGS Public Works immediately.
Providers must recognize that falsification of this certification carries significant penalties under Pennsylvania law, including civil penalties, contract termination, and three-year ineligibility to bid on Commonwealth contracts under 62 Pa. C.S. § 531. The certification is also subject to the provisions and penalties of 18 Pa. C.S. § 4904 regarding unsworn falsification to authorities. This certification requirement applies to all contractors pursuing the PLCB Security Guard Services contract, which seeks up to three qualified contractors to provide uniformed security personnel at Fine Wine & Good Spirits retail locations across three regions of Pennsylvania.
