About this file
This is a Request for Quote (RFQ) issued by the Phoenix Area Indian Health Service for FYSU Water Management Plan Development, Maintenance, Verification, and Legionella Testing Services. The solicitation (RFQ No. IHS1521987) includes numerous Federal Acquisition Regulation (FAR) and Health and Human Services Acquisition Regulation (HHSAR) clauses covering requirements such as electronic invoice submission, labor standards, equal opportunity, and various prohibitions on contracting.
Key compliance requirements include using the Department of Treasury Invoice Processing Platform for payments, adhering to service contract labor standards, and meeting provisions related to employee rights, trafficking prevention, and employment eligibility verification. The government may extend services up to 6 months beyond the initial contract period, and offers that do not fully respond to solicitation requirements may be rejected. The Phoenix Area Indian Health Service is tax-exempt under multiple Arizona state revenue statutes, and offerors must submit proposals with clearly marked Price, Technical, and Past Performance volumes following specific preparation instructions.
