SECTION 1

DESCRIPTION OF SERVICES

GENERAL SCOPE: The Contractor shall provide all personnel, labor, equipment, materials, supervision, and tools necessary to perform drayage of government-owned furnishings:

a. Family Housing Furniture: Transport family and unaccompanied housing loaner furniture between warehouse and housing units on Eielson AFB, Fort Wainwright, and off-base housing within the area defined in Appendix 2.

b. Unaccompanied Housing (Dormitories) Furniture and Equipment: Transport furniture and recreational equipment between the warehouse or other designated storage locations on Eielson AFB to the specified dormitory and dormitory room.

c. The contractor shall remove and dispose of refuse resulting from the transportation and assemble of new and repackaged furniture or recreational equipment according to commercial and industry standards, and all applicable federal, state, and local laws, regulations, and procedures to include OSHA Guidance 29 CFR part 1910 General Industry and AFI 91- 301 Para 9.1 - 9.5, AIR FORCE

OCCUPATIONAL AND ENVIRONMENTAL SAFETY, FIRE PROTECTION, AND HEALTH

(AFOSH) PROGRAM.

d. At contract start, the contractor shall complete any work left in progress by the previous contractor.

1. FURNISHINGS SET-UP AND CONNECTION: The contractor shall assemble/disassemble & setup all government-owned furnishings and recreational equipment, as well as setup and connect all appliances. Contractor will return items provided by FMS used to connect washer (hoses), dryers (vents and electric cords) and other supplies. FMS will not provide tools required for setup, connection or disconnections. Contractor shall maintain the correct type and amount of hand tools needed to assemble/disassemble furnishings.

1.1. MOVEMENT OF FURNISHINGS: All movement of furniture and equipment will be identified on the Request Order printed by the Enterprise Military Housing (eMH) system. Furnishings Management Section (FMS) personnel will provide a printed copy of the Request Order to the contractor identifying furniture for deliver or picked up.

1.1.1 The contractor will off-load and segregate returned furniture to the location specified by FMS personnel according to Request Order. FMS will inspect the returned furniture when all the furniture identified on the Request Order is in the designated location. The contractor liability for damage ends when FMS personnel accepts and verifies furniture returned by the contractor matches the Request Order.

1.1.2 When loading and off-loading more than one shipment at warehouse, FMS shall place each Request Order loaner furniture in segregated areas to match the Request Orders. The contractor will verify the segregated items barcodes match the furniture barcodes and deliver the furniture identified on the matching Request Order address. Contractor will pick-up furniture according to the Request Order for Pickup and segregate the furniture at the warehouse in locations specified by FMS. Contractor shall ensure furniture is clean before picking it up. If furniture is not clean, the contractor will notify FMS.

FMS will send an inspector to review the furniture and inform of the customer of ways to resolve the situation. FMS may instruct the contractor to cover and deliver the furniture to a dirty furniture portion

1.1.3 After initial preparation of the RO, FMS will be generate Furniture Condition. Any changes to condition(s) shall be initialed by the contractor and the customer, FMS or Airman Dorm Leader (ADL) personnel (when applicable). When required, the contractor shall be responsible for removing and reinstalling doors that affect movement of government furnishings in and out of on- and off-base residents or Unaccompanied Housing (Dormitories) quarters on Eielson AFB. The contractor with an ADL and appropriate 354 CES representatives shall conduct a joint and concurrent inspection of government facilities involved in the move in addition to property inspection as required. Existing damage to facilities shall be documented on a FMS Facility Condition form. Damaged items (other than normal wear and tear), shall be treated as exceptions and documented as added items on the RO. An attachment shall be used if required. The government may occasionally perform movement of minimum sized furnishing or individual ADL furniture pieces for residents when needed.

1.1.4 REPLACEMENT OF ITEMS: Any furnishings broken or damaged by the contractor during delivery shall be returned to the FMS warehouse and exchanged for serviceable items which shall be delivered the same day on original RO at no additional cost to government or occupant.

1.1.5 MISSED APPOINTMENTS: The contract is required to wait fifteen minutes after initial arrival at delivery/pick-up location. If resident cannot be contacted, the contractor shall attempt to contact occupant by means of telephone, ringing front door bell, and knocking on front and back door. If occupant cannot be located, the contractor shall contact FMS personnel or ADL, who will make one attempt to contact the service member at their duty section or one additionally provided telephone number. FMS or ADL will then authorize the contractor to depart the premise for their next appointment. The contractor shall be paid fifty percent (50%) of RO total for moves considered missed appointments. The Contractor shall not be paid if the time and name of the MHO or ADL who authorized the contractor to depart the premise is not indicated on the RO. If the contractor has been properly notified by FMS or ADL personnel of the date and time of delivery or pickup and misses the appointment, the contractor will have twenty percent (20%) deducted from the RO when move is rescheduled and accomplished.

1.1.6 VEHICLES: Each contractor vehicle shall have the name of the contractor neatly exhibited on each side of the cab. The vehicle shall be in satisfactory mechanical condition and shall present a neat and clean appearance. Vehicles shall be supplied with sufficient clean padding, covers, retraining straps and other protective equipment necessary to ensure safe transit, shipment, and delivery of items in an undamaged condition.

1.1.7 TRAFFIC MANAGEMENT OFFICE (TMO) CARRIERS: FMS or ADL personnel will not schedule a drayage move for the contractor if a TMO carrier will be performing work at the same address. The contractor shall not enter an occupant’s quarters if, upon arrival, a TMO carrier is working on the premises. If the contractor does not perform due to customer scheduling, the move shall be considered a customer missed appointment in accordance with (IAW) paragraph 1.1.5. If the contractor is late for a delivery or pickup resulting in an overlap with a TMO carrier, than the contractor will be listed as having a “Missed Appointment,” IAW paragraph 1.1.5.

1.1.8 PROPERTY INVENTORY AND INSPECTION: FMS or ADL personnel (when applicable) will provide the contractor with two copies of each RO and two copies of the Furniture Condition Form (FCF). For deliveries, the customer dates and signs both copies of the RO, one remains with the

Contractor and the one copy is returned to FMS or the ADL after the move is completed. For pickups, the customer signs both copies of the RO, one copy remains with the Contractor and the other copy is returned to FMS or the ADL. FMS or ADL will make a copy of the original FCF. The contractor and customer shall jointly and concurrently inventory and inspect items prior to loading into the contractors vehicle. The contractor and customer will signs both copies of the signed FCF. One copy of the FCF will remain with the customer and the other FCF copy is returned to FMS or the ADL. FMS or ADL will make a copy of the FCF at the contractor’s request. Inventory and inspection condition(s) other than normal wear and tear will be recorded. If damage is noted (e.g., item is deemed unserviceable), movement of item is stopped and the contractor shall contact FMS or ADL personnel (when applicable) immediately. FMS or ADL personnel (when applicable) will respond immediately to inspect the property.

The contractor is shall not attempt to resolve disagreements of conditions with the customer. If disagreements arise, the contractor will immediately notify FMS or ADL who will respond to the location and resolve the issue. FMS or ADL will release the contractor to complete other moves if the resolution will not be timely. The following condition codes shall be used and shall be recorded on pre-printed FCF:

1.1.9 DELIVERY & SETUP: The contractor shall deliver, assemble/disassemble and setup all furnishings, washers/dryers & appliances. The contractor shall jointly and concurrently inventory and inspect property with occupants for each delivery/pickup and both will sign RO and FCF.

1.1.10 CONTRACTOR WASHER/DRYER HOOK-UP: The contractor shall deliver washers and dryers per the RO. The contractor shall be responsible for supplying a certified installer to insure both water and electrical connections are correctly installed and the unit is setup correctly. This includes installing appropriate power cord to the dryer or stove, as necessary. Contractor shall assume all liability for damages that result from their negligence when hooking up connections to washers and dryers whether home is located on base or off base.

1.1.11 CONTRACTOR CLEANUP: The contractor shall remove, in or near residences and warehouse, any packing materials used with each move. Upon completion of the job, the contractor shall leave the work site and premises in a clean, neat condition. Recycling dumpsters are available for use.

1.1.12 NOTIFICATION OF SERVICES: Notification of pickup/delivery orders written against this contract will be issued by FMS or ADL personnel. FMS or ADL personnel (as applicable), will notify the contractor prior to 3:45 P.M., the day prior to date services are required. FMS or ADL personnel will designate whether drayage moves will be in the morning or afternoon. Morning moves shall be accomplished between 8:30 A.M. and 11:30A.M., except Saturdays, Sundays, and Federal Holidays.

Afternoon moves shall be accomplished between 12:30 P.M. and 3:30 P.M. except Saturdays, Sundays, and Federal Holidays. Upon receiving notification, the contractor shall perform drayage of items at

1.1.13 WEATHER CONDITIONS: The contractor shall perform work in variety of weather conditions.

The contractor is not required to perform work if weather temperature conditions are reported to be 40 degrees below zero, or colder (no wind chill factor will be used in this temperature). The contractor shall use the National Weather Service (NWS) Forecast Office, Fairbanks, Alaska Office for weather forecasts and advisories. The FMS will notify the contractor as soon as possible if delayed reporting is authorized or other closures of the installation. The contractor will not be liable for missed appointments due to items indicated in this paragraph.

1.1.14 DAMAGE LIABILITY: The contractor shall be responsible for all government furnishings while in his/her possession. The contractor shall be liable for any damages to government or privately owned quarters and for personal items damaged during performance of the contract. The contractor or customer shall immediately notify government personnel when damages are incurred. The contractor will be given the opportunity to inspect all reported damage claims.

1.1.15 REIMBURSEMENT: The contractor shall reimburse the government the replacement value for loss or damage to government furnishings. The contractor is responsible for replacement of damage to government, privatized housing or privately owned property on- or off base while performing services under this contract unless, in the opinion of the designated government representative, said damages can be satisfactorily repaired by the contractor to meet government standards. Any applicable reimbursement to the government will be processed through reduction of the invoice monthly by the contractor and agreed to by FMS or ADL personnel.

2.1. SECURITY REQUIREMENTS:

2.1.1. The contractor shall be responsible for obtaining all necessary clearances, cards, passes, decals, and other items required for access of contractor personnel and vehicles to Eielson AFB, Fort Wainwright or other areas where service will be performed. The contractor shall be required to apply for vehicle and personnel identification at the Pass and Registration Office, 354 SFS, Eielson AFB, AK, or Physical Security, Main Gate, Fort Wainwright. Upon completion or termination of this contract, the contractor shall ensure all vehicle and personnel identification are surrendered to office of issue.

2.1.2. Contractor employees shall wear name tags or clothing bearing the name of the company.

Employees shall wear a picture badge that contains the company name, and employee’s name. Picture identification shall be worn and displayed at all times.

2.1.3. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor as a potential threat to the health, safety, security, general wellbeing, or operational mission of the installation and its population. Contractor personnel shall be courteous and professional, present a clean, neat appearance and by easily recognized.

2.1.4. The contractor shall maintain a current listing of all employees. The list will include employee names and date of birth. This list shall be validated by the contract manager and provided to FMS, the Contracting Officer and to 354 SFS at Eielson AFB, AK at the contract start date. An updated listing shall be provided upon change of employees.

2.1.5. The Contractor is responsible for protecting all resident Personally Identifiable Information (PII) provided as necessary to complete the furniture move. Examples of PII include name, address, telephone numbers (home and or mobile), etc.

2.1.6. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL

COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER. During times of crisis or national emergency, the contractor shall continue to provide services as scheduled, with minimal disruption of services.

2.2. SAFETY & VOLUNTARY PROTECTION PROGRAM:

2.2.1. The contractor shall perform work in a safe manner as required by OSHA Safety and Health requirements.

2.2.2. The contractor shall provide a verbal report to the Contracting Officer (CO) as soon as possible after each occurrence of damage to government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact.

Safety, Security, Identification The contractor shall make all the necessary requirements with SFS to ensure contract personnel have the proper security clearance and passes to perform work on installation. Personnel shall wear identifying clothing bearing the company name for identification and present a neat & clean professional appearance and attitude. Contractor shall not employ persons on this contract that present a security, safety, or well-being risk to installation personnel. Contractor vehicle shall present the company name neatly and clearly on the outside of the vehicle.

APPENDIX 1

Abbreviations, Acronyms and Definitions

AFI (Air Force Instruction) - a published instruction for members of the United States Air Force intended for use by active duty, guard, and reserve members and associated civilians.

RO (Request Order) - a document issued by FMS or ADL and signed by the contractor’s assigned delivery personnel, acknowledging that specified goods have been received on board as cargo for conveyance to a named place for delivery to the consignee who is identified.

CO (Contracting Officer) - a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the CO acting within the limits of their authority as delegated by the CO.

Contractor - Refers to both the prime contractor and subcontractor. The prime Contractor shall ensure that his/her subcontractors comply with the previsions of the contract.

ADL (Airman Dormitory Leader) - A system where permanent party enlisted dormitories are managed by a consolidated or centralized office.

CH (Community Housing) - Non-government owned housing, typically located off base.

DoD (Department of Defense) - an executive branch department of the federal government of the United States charged with coordinating and supervising all agencies and functions of the government concerned directly with national security and the United States Armed Forces.

FH (Family Housing) - A building, whether located on or off base, used as residential dwelling.

FY (Fiscal Year) - Federal fiscal year (1 October-30 September).

FMS (Furnishings Management Services).

PII (Personally Identifiable Information) - Any representation of information that permits the identity of an individual to whom the information applies to be reasonably inferred by either direct or indirect means. Further, PII is defined as information: (i) that directly identifies an individual (e.g., name, address, social security number or other identifying number or code, telephone number, email address, etc.)

QA (Quality Assurance) personnel – FMS and ADL personnel trained and approved by 354 Contracting Squadron to preform Quality Assurance responsibilities pertaining to this contract and any approved modifications.

APPENDIX 2

Family Housing Family and Unaccompanied Housing Loaner Furniture

3.1. Loaner Furniture moves shall be done by kit:

3.1.1. Loaner Furniture Moves: FMS will notify the contractor by 12 PM the last business day weekly of known move orders for the following week. The contractor shall provide vehicles and personnel to complete the weekly moves. FMS personnel will notify the contractor before 3:45 P.M.

daily of changes for the next business day. At time of notification, FMS personnel will identify number of drayage moves in the morning and afternoon and any appointment times. Morning moves shall be accomplished between 8:30 A.M. and 11:30 A.M., except Saturdays, Sundays, and Federal Holidays. Afternoon moves shall be accomplished between 12:30 P.M. and 3:30 P.M. except Saturdays, Sundays, and Federal Holidays. Upon receiving notification, the contractor shall perform drayage of kits at prices set forth in bid schedule, unless paragraph 3.1.2 or 3.1.3 apply.

3.1.2 Emergency or unscheduled moves will be priced at 1.5 times the price set forth in bid schedule.

Emergency moves may be on Saturdays, Sundays or Holidays.

3.1.3 The MHO will offer customers appointment times for moves. If a time is not specified on the RO, the contractor will deliver and pickup furniture within the time allotted for an AM or PM RO.

Appointment times will be spaced according to the following schedule.

Off-base delivery and pickup - 1.5 hours per Request Order On-base delivery and pickup - 45 minutes per Request Order If the contractor completes an appointment early, the contractor is free to complete any other RO according to AM or PM requests.

3.1.4 Delivery Area:

Salcha: 10 miles south of Eielson AFB South gate and 2 miles off Richardson Hwy.

North Pole: Nordale Rd to Chena River and all roads west of Nordale Rd and south of Chena River along Badger Rd.

APPENDIX 3

Unaccompanied Housing (Dormitories) Typical UH Furnishing Items

4.1. Unaccompanied housing moves shall be done by item (when applicable), or by half or full day moves. ADL personnel will coordinate with FMS of required full or half day moves, generally within 14 business days. FMS will notify the contractor of full and half day dorm moves at least 5 business days before the move. The contractor provide additional vehicles and personnel to complete Full and Half Day Dormitory moves within 5 business days of the move. All moves will be at the schedule identified in Appendix 3, paragraph 3.1.1, when the following requirements are met:

4.1.1. INDIVIDUAL DORMITORY MOVES: “Per Item” is defined as any items that are moved individually or when work is performed for 20 items or less moved from rooms or common areas, all other work performed shall be a “Half Day” or “Full Day” move as defined below.

4.1.2 HALF DAY DORMITORY MOVES: “Half Day” is defined as five full rooms or less or 20 items or less in a common area. Contractor is expected to complete the move as scheduled hours.

4.1.3 FULL DAY DORMITORY MOVES: “Full Day” is defined as six or more rooms or as more than 20 items in a common area. Contractor is expected to complete the move between 7:30 AM and 4:30

PM.
