This Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) details a chiller motor repair service for the USDA National Centers for Animal Health (NCAH) in Ames, Iowa. The project involves repairing a specific YORK chiller motor (Serial # W20000379-EK, HP 1215, 4160 volts) that has shorted to ground and requires bearing replacement. The contractor must provide manufacturer-qualified repair services, including motor removal, transportation, repair, and reinstallation, with a 30-day performance period.

Key requirements include submitting proof of insurance within 5 calendar days of award, scheduling motor removal and return with the Contracting Officer's Representative (COR), using a contractor-provided crane for motor handling, transporting the motor to and from a repair facility, replacing bearings, testing motor functionality, and performing a chiller start-up and operational verification. The repair must be completed within 25 calendar days of Notice to Proceed, with the work taking place at the NCAH campus. Security requirements mandate that all contractor personnel be US citizens with Real ID, check in at the main gate, and remain escorted while on site.
