RFP 400-26-84342, Attachment F Technical Proposal Template
Instructions: Please provide responses to the questions in each section below. Where appropriate, supporting documentation may be referenced by specific page and/or paragraph number(s). If any of the responses contain confidential information, as defined in IC 5-14-3, please reference the attached confidential material and separate from the rest of this response document. A redacted version of this document must be submitted if confidential information is included within this response template.
PART 1: MANDATORY REQUIREMENTS
1. Please indicate (yes/no) if the respondent has experience within the field of pediatric audiology, which includes hearing aid technology and relationships with hearing aid vendors. In addition, please indicate if the respondent has previous experience in procuring, dispensing, and billing amplification. If yes, please provide a description of the requested experience to demonstrate the requirement is met. Please be advised that a “no” response may result in the disqualification of the proposal from consideration of award.
PART 2: TECHNICAL QUESTIONS
1. Fund management – How would assurance of accurate fund management be obtained? Share ideas for processes related to POs for hearing aids, obtaining cost participation dollars, management and disbursement of dispensing fees and invoicing IDOH for reimbursement.
2. Application process – What is your experience with processing applications for programs similar to HAAPI? How would your entity propose applications be processed? Share ideas on timelines, documentation, data management, what staffing is available to manage this process.
3. Pediatric audiology knowledge and background – Share experience and knowledge of your staff in the area of audiology, including reading/reviewing audiograms, hearing aid fitting and dispensing, and best practices for pediatric amplification. Share ideas for gaining this expertise if not already a part of your entity.
4. Professional relationships – What professional relationships does your entity currently have with hearing aid vendors and Indiana audiologists? Share ideas on keeping these relationships strong as the HAAPI administrator as well as building new relationships. Inclusion of previous relationships is also valid- share how these relationships would be rekindled.
5. Public Relations – What experience does your entity have in public relations for other programs? How would these experiences help as the HAAPI administrator? Share experience related to creation and maintenance of websites, form creation (including brochures), and exhibit booths and other ways of promotion. What ideas do you have for implementation as the HAAPI administrator?
6. Reporting – What reporting have you overseen in the past? How would this benefit the HAAPI program should your entity be the HAAPI administrator? What data points would you find valuable to share with IDOH in quarterly reports?
7. Data management – Share your entities experience with record keeping for programs. Propose how information would be kept related to participating audiologists, vendor records including price lists and orders, application documents, receipts and invoices. How would IDOH have access to this information?
8. Staffing – Share what staffing your entity would have available to oversee the administration of HAAPI. Specifically: availability for additional duties, how to balance other duties to ensure consistent time allocated to HAAPI administrative needs, skills of staff useful to the work
9. Hearing Aid refurbishing program – share ideas on how to implement this process with hearing aids previously purchased with HAAPI funds and returned by families to the program.
