About this file

This document contains a past performance questionnaire for contractors bidding on a pond liner and kettle modification project. The federal agency soliciting bids is the U.S. Fish and Wildlife Service for their Ouray National Fish Hatchery. The project involves installing new 45 mil reinforced flexible polypropylene pond liners in six ponds and reconfiguring piping features according to provided drawings. Reinforced concrete curb will also be installed in existing catch basins and along kettle stair sections. Respondents are asked to evaluate a contractor's past performance on categories such as personnel quality, quality control, problem solving, compliance, responsiveness, submittal timeliness, and resolution of punchlist items or warranty calls. The questionnaire requests information on the evaluator and contract to aid in resolving any discrepancies between contractor and evaluator performance assessments.
