About this file

This is a Vendor Disclosure Statement form issued by the State of Colorado on July 19, 2024, applicable to the Methane Seep Program contract. The form requires vendors to disclose whether any services under the contract or related subcontracts will be performed outside the United States or Colorado, in compliance with Colorado Revised Statute 24-102-206. If services are anticipated to be performed outside these jurisdictions, vendors must identify the specific countries and states where work will occur and provide a detailed explanation of the business necessity or advantages for performing work outside the United States or Colorado. The form requests the contract routing number and vendor name for tracking and identification purposes.

The disclosure requirement applies to all state contracts and solicitations for services executed after August 3, 2007, with specific exemptions for contracts related to Medicare, the Colorado Medical Assistance Act, the Children's Basic Health Plan Act, and the Colorado Indigent Care Program. Completion and return of this form to the contracting agency is mandatory for vendors that have not already provided this information in the Statement of Work, ensuring transparency regarding offshore or out-of-state performance activities.
