About this file
This document is a Contract Amendment (Amendment #5) for Contract #CTR055722 between the State of Arizona's Department of Child Safety (DCS) and an unnamed contractor. The amendment involves modifications to an existing contract, primarily focusing on ridesharing provisions and invoicing requirements for both the Department of Child Safety and the Arizona Department of Juvenile Corrections (ADJC).
The amendment primarily clarifies and updates transportation service requirements, specifically prohibiting ridesharing for children and young adults (ages 0-21) while permitting it for adults over 21. It establishes detailed driver qualification standards, including requirements for valid driver's licenses, car insurance, background checks, driving history, CPR certification, and drug testing. The amendment also updates invoicing procedures, specifying that invoices for DCS services must be submitted via the DCS Case Management Welfare System between the 1st and 15th of each month, while ADJC invoices should be submitted to a specific accounting email address according to Exhibit F of the contract. Additionally, the amendment updates indemnification language and replaces the Uniform Terms and Conditions from Version 9 to Version 10.2.
