About this file

This document is an Affirmation and Disclosure Form associated with the Ohio Department of Transportation (ODOT), specifically involving a contractor located in Marietta, Ohio. The form requires the contractor, Ben Lang from a company at 414 Muskingum Drive, to affirm compliance with Executive Order 2022-02D, which prohibits purchasing services from Russian companies or institutions. The document mandates that all services and state data must be performed and maintained within the United States, with potential contract termination if these requirements are not met.

The contract appears to involve services at ODOT District 10's Rest Area on I-77 Northbound at Mile Marker 3.56, with the contractor's primary location in Marietta, OH. While specific service details are not extensively outlined, the document emphasizes strict geographical restrictions on service delivery and data management. The contractor must disclose any changes in service location before, during, or after contract execution, and ODOT reserves the right to recover funds if any services are performed in violation of the specified geographical constraints. The form was executed on 4/16/24 by Ben Lang in his capacity as Maintenance Manager.
