About this file

This document is a Bid Sheet (Attachment A) for the State of Florida's Department of Management Affairs (DMA) soliciting tree trimming and herbicide application services, specifically the DMA-ITB-347-SCWTS Tree Trimming & Herbicide Services contract. The solicitation is for a 3-year contract period with specific service requirements including quarterly tree trimming (4 times per year), quarterly inspections and reporting with photos (4 times per year), herbicide applications for Cogon Grass (2 times per year in fall and spring), and herbicide applications for fence lines (4 times per year concurrent with trimmings).

The bid sheet requires vendors to provide unit pricing for each service type, with the total annual cost to be calculated by multiplying the unit price by the specified annual quantity. Bidders must complete the form, providing their company details, federal identification number, contact information, and an authorized signature certifying compliance with all solicitation terms and conditions. The pricing structure allows for transparent cost evaluation across different service components, with a clear expectation of four tree trimmings, four inspection reports, two Cogon Grass herbicide applications, and four fence line herbicide applications annually over the contract's three-year term.
