About this file

This is an experience references submission form (Exhibit C) prepared by the City of Detroit Office of Contracting and Procurement for a request for proposals (RFP) regarding HVAC replacement services at Hart Plaza. The form requires respondents to submit references in Excel format documenting up to five relevant projects completed within the five years preceding the RFP publication date. For each reference, respondents must provide comprehensive client information including contact names, email addresses, and phone numbers; original contract terms in months; service start and end dates; starting and ending contract amounts; detailed descriptions of services provided aligned with the Scope of Services; the number of sites or facilities serviced; and disclosure of any citations or violations received during service delivery. Respondents with more than five qualifying references should select those projects that best demonstrate their capability to perform the requested HVAC design-build services.

The form includes mandatory supplemental documentation requirements where respondents must submit explanations in portable document format (PDF) to address specific conditions. These conditions include written explanations for any service contracts with early termination or extensions beyond the original term; detailed justifications for changes in contract value between service start and end dates; comprehensive lists of all sites and facilities serviced, including names, addresses, and service types provided at each location; and detailed documentation of any regulatory citations or violations, including the issuing authority, violation date, description, and final disposition. Failure to complete the form with all required information may result in the submission being deemed incomplete. The respondent must ensure that all client contact information is current and that the designated contact person possesses comprehensive knowledge of the services provided.
