About this file

This document is an Attachment A - Insurance Information from the Colorado Department of Corrections (CDOC) detailing comprehensive insurance requirements for contractors. The document outlines mandatory insurance coverage types and minimum coverage limits for entities seeking to contract with the state, including Workers' Compensation, General Liability, Automobile Liability, Protected Information, Professional Liability, and Crime Insurance. Contractors must provide specific insurance certificates within 7 business days following the contract's effective date, with coverage amounts potentially negotiable depending on the services provided.

The insurance requirements are extensive and tailored to protect the state from various potential risks, with minimum coverage limits typically set at $1,000,000 per occurrence and $2,000,000 in general aggregate for most policy types. Specific provisions include naming the State as an additional insured, ensuring primacy of coverage, preventing policy cancellation without notice, and including subrogation waivers. Contractors must carefully evaluate their specific service context to determine which insurance types are required, with guidance provided through a series of qualifying questions about employee status, information access, and service types. The document emphasizes that evidence of insurance is mandatory before a contract or purchase order will be issued.
