About this file
Exhibit E is a Contract Performance Disclosure Statement from the Colorado Department of Corrections (CDOC) related to the Warehouse Commodities #2 solicitation for Baking, Cereal & Dried Goods. The document is a standard form requiring vendors to disclose where services will be performed, with specific emphasis on identifying any work conducted outside of the United States or Colorado. The form is part of the procurement process for a contract to supply commodities to six warehouse locations throughout Colorado, and is designed to ensure transparency about contract performance locations.
The disclosure statement specifically requires vendors to list all counties and states where contract services will be performed, and if any services are anticipated to be performed outside of Colorado or the United States, the vendor must explain the necessity or advantage of doing so. The form is not required for certain types of contracts, including those under Medicare, medical assistance programs, children's health plans, or construction contracts. Vendors must complete and sign the form as part of their solicitation response, providing details about their proposed contract performance locations and any potential out-of-state or out-of-country work.
