APPENDIX B
SCOPE OF SERVICES
LIMITED ASBESTOS & LEAD-BASED PAINT SURVEY
HURRICANE IRMA REPAIR WATERFRONT
CG SECTOR KEY WEST, KEY WEST, FL
Synopsis
The scope of work for this project consists of (a) An asbestos containing material (ACM) and lead-based paint (LBP) investigation and analysis, (b) Project administration and coordination, and (c) Report preparation.
Background
The concrete bulkhead on the south side of Pier D2 will be repaired on the area indicated in the aerial map included at the end of this appendix. There may also be paint stripping on some areas of the bulkhead that will be disturbed during the concrete repair work. A LBP and an ACM assessment are required for all those areas that will be directly impacted by this project.
It is suspected that due to the age of the bulkhead, the concrete could contain asbestos residues. In addition, the paint stripping applied to some areas of the concrete bulkhead and adjacent concrete curbs could contain lead.
Scope of Services
1. All Asbestos Surveys shall be conducted by a licensed professional to perform asbestos work, and in accordance with all Federal, State, and Local laws, rules, regulations and standards.
2. If available, it is recommended that project plans/drawings be reviewed prior to the site visit.
3. Conduct a site inspection of the bulkhead on the south side of Pier D2. The bulkhead concrete shall be sampled and must have all suspects accessible friable and non-friable Asbestos
Containing Material (ACM) identified, documented and quantified.
4. Indicate all areas of homogeneous suspect material. This should be identified on a site plan drawing. These areas should also be clearly delineated in the written report. The final report shall contain a site plan diagram indicating sampling locations, denoted with an identification number corresponding to the representative sample number.
5. ACM shall be identified as friable and non friable under the following categories:
Surfacing Material
Thermal System Insulation
Miscellaneous Materials
Use the hazard ranking system to indicate ACM condition, disturbance potential, hazard rank and response priority.
6. ACM shall be further subcategorized as stipulated in NESHAP’s 40 CFR Part 61 by, Category I Non-Friable ACM
Category II Non-Friable ACM
Regulated ACM
7. Identify areas where ACM is suspected but cannot be sampled and indicate why it was not sampled on the floor plan. Diligent effort must be made to sample all areas except where clearly impossible.
8. All Quality Assurance and Chain of Custody protocols must be adhered to. All documentation shall be fully supported in the final report.
9. Applying the AHERA protocol, collect and analyze bulk samples of all ACM in each homogeneous area. Asbestos shall be analyzed /identified by POLARIZED LIGHT
MICROSCOPY (PLM) by an NVLAP accredited laboratory. The analysis of bulk samples must identify the complete matrix of sample constituents.
10. Report shall be formatted in the following manner:
(a) Background inspection and results narrative.
(b) Hard copy of plan with sample locations. Return a digitized copy of the building plan with the sample locations indicated.
(c) Chain of Custody Forms.
(d) Laboratory Analytical Report.
(e) Table with hazard ranking system indicating ACM condition, disturbance potential, hazard rank and response priority.
(f) Copy of current Asbestos Business Organization and Asbestos Consultant License.
(g) Copy of applicable current certifications for personnel performing survey and report.
(h) Copy of current Laboratory NVLAP certification with applicable scope.
11. If the Consultant or its subcontractor discovers a location that requires immediate action such as a fiber release or significantly damaged ACM, the Consultant shall notify the CEU
Contracting Officer immediately.
12. Perform a LBP survey of the suspect areas of lead containing paint stripping and analyze for total lead.
13. The LBP Survey shall be conducted by a consulting licensed professional to perform LBP surveys, and in accordance with all Federal, State, and Local laws, rules, regulations and standards.
14. Prepare report of results.
Coordination
Coordinate all site visits and work schedules with the unit point of contact at CG Sector Key West prior to the date required. Notify a minimum of 3 (three) days prior to the start of the fieldwork.
Fieldwork shall not interfere with the normal daily operations of the Station.
Access to Facility
Access to U.S. Coast Guard Sector Key West is through a Navy controlled access gate. The Navy requires all contract personnel accessing the facility to have credentials under the RAPID Gate program managed by Eid Passport, Inc. Each contracting and subcontracting company and venders shall be enrolled in the program. Every employee of those companies who accesses the facility must be registered in the program. Fees for enrollment and registration shall be paid by the contractor. For instructions on enrollment and registration in the RAPID Gate program contact Eid
Passport customer service at (877) 727-4342. The contractor needs to maintain this access through the warrantee period, and not just the construction period.
Cleanup
The work area will be restored to a similar condition as encountered prior to the start of the fieldwork.
Safety
All site personnel shall be trained and certified under OSHA 1910.120 requirements. Level D protection is believed to be appropriate; but existing site conditions shall be verified prior to the start of any fieldwork. The work site shall be considered a construction zone, requiring proper attire including hardhats and safety shoes.
