About this file
This is a Schedule B pricing document for the Michigan Respiratory Protection Program RFP at MVH (presumably Michigan Veterans Home). The document establishes the pricing structure and cost submission requirements for vendors responding to the respiratory protection program solicitation. Bidders are required to submit detailed pricing for all respiratory protection equipment, supplies, and services as outlined in the scope of work, including initial equipment procurement, replacement cartridges, filters, fit-testing supplies, training materials, and any ancillary services necessary to maintain a comprehensive respiratory protection program. The pricing schedule requires vendors to itemize costs by category, specify unit prices, and provide total project costs for the initial implementation period and any ongoing annual maintenance or supply replenishment phases.
The pricing submission must include all costs associated with the respiratory protection program with clear line-item breakdowns to facilitate evaluation and comparison among bidders. Vendors should specify whether pricing includes delivery, installation, training, and support services, and clarify any volume discounts or pricing adjustments that may apply based on program scale or duration. The document requires vendors to certify that their pricing is firm for the initial contract period and to identify any costs that may escalate in subsequent years or renewal periods, including justification for any proposed increases.
