About this file
This is a Certification of Debarment form required by the U.S. Department of Agriculture for lower tier covered transactions, applicable to the San Felipe Del Rio Consolidated Independent School District's asbestos abatement procurement in Texas. The certification requires prospective bidders to verify that neither they nor their principals have been debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in federal transactions. Bidders must select one of two certification options: either certify compliance with debarment requirements or attach an explanation if unable to certify. The form requires completion by an authorized representative with signature and date.
The certification mandates that bidders provide immediate written notice to the contracting entity if they subsequently learn that their certification was erroneous at submission or becomes erroneous due to changed circumstances. Federal and state penalties exist for vendors and districts that knowingly enter into contracts with suspended or debarred persons. This form implements Executive Order 12549 and regulations codified in 7 CFR Part 3017, Section 3017.510, and serves as a compliance requirement prior to contract award for the asbestos abatement services procurement.
