About this file
This document is an attachment to the Rural Health Transformation Program (RHTP) solicitation issued by the State of Utah Division of Purchasing, with oversight by the Utah Department of Health and Human Services (DHHS). The attachment establishes mandatory minimum requirements and technical scoring criteria for vendor evaluation. The RHTP is a five-year federally funded initiative from the Centers for Medicare & Medicaid Services (CMS) designed to strengthen health systems in rural communities. The program operates across 25 of Utah's 29 statutory rural and frontier counties, excluding Salt Lake, Utah, Davis, and Weber counties. The selected vendor must deliver a Project Plan within 30 days of contract execution and a comprehensive Evaluation Plan within 60 days. The contract term extends through 2030, with annual funding contingent upon demonstrated program outcomes and CMS discretion. The program framework is built on four strategic pillars—Making Rural Utahns Healthy, Workforce Development, Innovation and Access, and Technology Innovation—operationalized through seven integrated initiatives and 24 key activities designed to transform rural healthcare delivery and drive measurable improvements in health, coordination, and financial sustainability.
Vendors must satisfy all mandatory minimum requirements before advancing to technical evaluation, including demonstrated experience with Results Based Accountability (RBA) frameworks, commitment to serve only the designated 25 rural counties, compliance with federal CMS requirements and the Stevens Amendment, capacity to meet strict deliverable timelines, and absence from federal or state debarment lists. The contract is valued at $195.7 million in Year 1 federal CMS funding, with continued annual funding dependent on performance outcomes and CMS discretion. Vendors must be registered legal entities—for-profit, non-profit, or government—with valid operational status and must register as active vendors with Utah State Purchasing within 14 business days of receiving a Notice of Intent to Award. Technical evaluation encompasses seven criteria areas: evaluation methodology and RBA framework implementation, sustainability and fiscal stewardship aligned with House Joint Resolution 101 requirements, performance monitoring and quarterly reporting, targeted population and tribal area strategies, data dissemination and communication capabilities, federal compliance and budgeting oversight, and legal entity documentation. In strict alignment with H.J.R. 101, all RHTP initiatives must prioritize non-recurring upgrades and transition into self-sustaining, market-driven structures that create no ongoing financial obligations for the State of Utah.

Instructions:
Vendor must answer yes or no to each minimum mandatory requirement. The vendor and services proposed must meet (answer yes) all minimum mandatory requirements to advance to the Technical Criteria Evaluation.

· Federal Compliance and Restrictions: Does the offeror agree to comply with all federal CMS funding requirements and the Stevens Amendment guidelines for public-facing materials?
· Strict Deliverable Timelines: Can the offeror guarantee the capacity to submit the initial Project Plan within 30 days of the contract effective date and the comprehensive Evaluation Plan within 60 days of the contract effective date?
· Federal and State Standing: The applicant must not be debarred, suspended, or otherwise excluded from receiving federal or state funding.
· Legal Entity Status and State Vendor Registration: The applicant must prove legal operational status as a recognized entity (for-profit, non-profit, or government) and formally attest to their eligibility and intent to register as an active vendor with Utah State Purchasing within 14 business days of receiving a Notice of Intent to Award.

· Describe your approach to structuring the comprehensive Evaluation Plan so that it effectively encompasses outcome evaluation for the four Strategic Goals, performance evaluation for the seven initiatives, and pilot evaluations.

2) Sustainability and Fiscal Stewardship
· How will you use evaluation information to propose strategic roadmaps that enable State, tribal, and local safety-net entities to sustainably absorb and scale impactful activities?
· What specific methods will you use to ensure these transition models prioritize non-recurring upgrades and transition into market-driven structures, aligning strictly with H.J.R. 101 to avoid ongoing financial obligations for the State?
3) Performance Monitoring and Reporting
· Describe your process for developing the comprehensive Measurement Framework within 30 days of the Evaluation Plan submission, ensuring it contains qualitative and quantitative metrics to track the performance of RHTP interventions.
· How will your team manage quarterly progress monitoring to generate actionable performance improvement recommendations for RHTP staff, particularly for the reporting period ending April 30?
4) Targeted Populations and Tribal Areas
· How will your evaluation leverage available data to pinpoint high-needs populations and identify their associated service and access gaps?
· What specific strategies will you employ to monitor progress in tribal areas, ensuring that the implemented activities adequately address their unique needs?
5) Data Dissemination and Communication
· Describe your operational capacity to rapidly produce ad hoc executive summaries and presentation materials for leadership and stakeholder audiences upon request.
· What is your strategy for translating complex evaluation data into accessible communication products—such as success stories, policy briefs, and case studies—suitable for CMS and legislative stakeholders?
6) Budgeting and Federal Compliance
· How will your fiscal oversight practices ensure funds are obligated no later than June 30 for budget years 2-5 to prevent the recoupment of funds by DHHS?
· How will you ensure strict compliance with the Stevens Amendment regarding the mandatory acknowledgment of federal assistance on all public-facing materials?
7) Legal Entity Status and State Vendor Registration: The applicant must prove legal operational status as a recognized entity (for-profit, non-profit, or government) and formally attest to their eligibility and intent to register as an active vendor with Utah State Purchasing within 14 business days of receiving a Notice of Intent to Award.
