About this file

This is a Scope of Work Deliverables document for the Rural Health Transformation Program (RHTP), a federally funded initiative administered by the Utah Department of Health and Human Services in partnership with the Centers for Medicare & Medicaid Services (CMS). The contractor must deliver comprehensive project planning, evaluation efforts, monitoring activities, and reporting materials spanning the five-year implementation period through October 2031. Key deliverables include an initial project plan and timeline due within 30 days of contract execution, annual project plan updates due by July 1, a multi-year evaluation plan with baseline, midline, and endline evaluation reports due at specified intervals, and quarterly progress reports due November 7, February 7, and May 7 annually. Additional requirements include development of a measurement framework with key performance and outcome metrics, annual process monitoring with performance improvement recommendations, and executive summaries and presentations prepared as requested by the state. The final report summarizing all contractor efforts and achievements is due December 30, 2031, coinciding with the program's anticipated completion date.

The deliverables encompass evaluation and monitoring components designed to assess the RHTP's impact on rural populations and health systems across four strategic pillars and seven integrated initiatives operating in 25 of Utah's 29 counties. The contractor must develop baseline, midline, and endline evaluation reports with inception drafts and final versions, conduct deep-dive evaluations on selected innovative activities, and produce impact stories, case studies, policy briefs, and sustainability recommendations suitable for CMS and other stakeholder audiences. The scope emphasizes adherence to Utah House Joint Resolution 101's fiscal stewardship principles, requiring that RHTP initiatives prioritize non-recurring upgrades and transition operations into self-sustaining, market-driven structures. The program is federally funded through CMS with $195.7 million awarded for Year 1, with continued annual funding contingent upon program outcomes, fund availability, and CMS discretion through the anticipated 2030 completion date.
