About this file
This is a Master Services Agreement between Maxim Healthcare Services, Inc. (Contractor) and the Wisconsin Department of Corrections (DOC) for managed services provider (MSP) services and staffing services. The agreement, revised February 1, 2019, establishes Maxim as the primary managing vendor for healthcare staffing services across DOC facilities, with the Department of Health Services (DHS) having the ability to piggyback on the contract for nursing staff.
The contract covers healthcare staffing services including Registered Nurses, Licensed Practical Nurses, Certified Nurse's Aides, Medical Assistants, Medical Records Technicians, Respiratory Therapists, Nurse Practitioners, and Phlebotomists. Maxim provides both direct MSP services and manages third-party staffing suppliers through a centralized system. Pricing is established through hourly rates listed in Schedule C, with the total cost proposal for all positions at $14,189,946.00. The MSP fee is calculated at 4% of the total amount billed to DOC for services. The contract includes provisions for overtime rates (additional $14.00 per hour), weekend rates (additional $1.50 per hour), holiday rates (additional $14.00 per hour), and on-call services at 1.5 times the regular billing rate. Payment terms require weekly invoicing with 30-day payment cycles, and DOC requires positions to be filled within 30 days of request or the department may seek alternative providers.
