Bidders must indicate their own certification status if applicable, provide certification agency details including effective and expiration dates, and document all outreach efforts undertaken to identify and include MWSBEs in their proposals. Additionally, companies must describe the outreach efforts they will implement during the contract performance period to maximize MWSBE participation.
The form requires bidders to list all subcontractors they intend to engage, including a detailed description of work or materials to be provided, designation categories (M, W, and/or S), City vendor numbers, and calculated utilization percentages for each category. The City maintains a strong commitment to MWSBE inclusion in its contracting process and recommends that bidders exhaust all reasonable efforts to identify viable subcontracting opportunities with certified enterprises. This utilization form must be completed and submitted as part of the company's formal proposal for contract consideration.
