About this file

This is a Cost Proposal Form for a contract opportunity issued by the Ohio Department of Education and Workforce (DEW) for Assessment Forensics and Security Services. The contract encompasses four primary deliverables: Design Methodology, Onsite Investigation Audits, Web Monitoring, and Summary Findings and Recommendations Report. The contract period is divided into two phases: FY27 (upon award until June 30, 2027) and FY28-FY29 (July 1, 2027 through June 30, 2029). Each deliverable is priced as a single unit cost with cost extension calculations required for both fiscal year periods.

Offerors must submit cost proposals exclusively within the designated Cost Proposal envelope, with inclusion in technical proposals resulting in potential disqualification. Each offeror is required to propose a detailed cost model narrative for Deliverable 2 (Onsite Investigation Audits), accounting for varying complexity levels based on engagement specifics as detailed in pages 8-9 of the Request for Proposal Scope of Work. DEW reserves the right to review cost proposals during evaluation to reject proposals deemed excessively costly or to bypass subsequent evaluation phases and award based solely on preceding phase scores. Offerors must provide unit costs and extended cost calculations for all four deliverables across both contract periods.

Cost Proposal Cost proposals must be submitted within the Cost Proposal envelope only. Inclusion within the Offeror’s technical proposal may lead to disqualification. At any time during the evaluation process, DEW may review cost proposals to determine whether any proposals should be rejected due to excessive cost. Alternatively, DEW may choose to bypass any or all subsequent phases and award based solely on its scoring of the preceding phases.
