About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico, for the East Road 14" Water Line Replacement project. The form serves as a required certification document for bidders participating in this procurement opportunity. The project involves the replacement of approximately 6,700 linear feet of existing 14-inch steel water transmission line installed in 1949 along NM-502. Due to age-related deterioration and frequent leaks, the existing pipeline will be replaced with new 14-inch ductile iron pipe. This line is a critical transmission main conveying water from Otowi Well #4 to the Townsite area of Los Alamos.

The certification form requires bidders to attest that neither they nor any of their principals are debarred, suspended, proposed for debarment, or otherwise ineligible to conduct business with government entities. Bidders must certify they have not been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, bribery, or falsification of records within the preceding three years. Additionally, bidders must confirm they have not violated contract provisions, experienced unsatisfactory performance on prior contracts, violated ethical standards, are not current or former County employees, are not immediate family members of County employees or officials, and have not had any public contracts terminated for cause or default within three years. Any bidder unable to certify these statements must provide written explanation. Falsification of any statement in the form constitutes grounds for non-consideration or rescission of contract award. Bidders must also confirm that all proposed subcontractors are not currently suspended or debarred from conducting business with any governmental entity.
