About this file
This is an Application Form for the Delaware Opioid Abatement Community Grant Program (DOSC) General Grant Category, administered by the State of Delaware for organizations addressing opioid and substance use disorders. The program awards grants of $30,000 or more to qualified organizations for initiatives with a performance period of up to two years. Eligible applicants must be formally registered, legally incorporated entities (nonprofit, for-profit, or governmental) operating within Delaware with a minimum of two years of consistent program delivery and documented operational stability. The application requires organizations to submit comprehensive documentation including organizational information, board member or ownership details, project descriptions, target population demographics, evaluation frameworks, budget narratives, and evidence of community engagement and equity commitments. Applicants must select at least one opioid abatement and remediation strategy from categories including treatment and recovery supports, prevention, harm reduction, first responder assistance, and research initiatives. The application process requires submission of independent financial reviews or audits conducted by certified professionals unaffiliated with the organization, along with proof of financial stability from the previous fiscal year.
The requested grant amount must be $30,000 or more and must exactly match the total amount listed in the attached project budget worksheet. Organizations must provide their current year operating budget and a three-year funding history documenting all financial support received. The application emphasizes organizational capacity assessment, including mission statements, core activities, implementation experience, and demonstrated track record of successfully executing similar projects. Applicants must detail their project leadership team qualifications, identify external partnerships and stakeholders with specific roles and responsibilities, and describe long-term sustainability plans beyond the grant period. The program prioritizes initiatives addressing treatment access, harm reduction, prevention, and recovery support networks while advancing equity and removing barriers to service access for historically underserved populations. Community engagement and shared decision-making throughout the project lifecycle are required components of the application.
