About this file
This document contains the standard contract forms (Forms A through E) required by the Board of School Commissioners of the City of Indianapolis for Indianapolis Public Schools (IPS) contractors and vendors. The forms establish mandatory disclosure and compliance requirements for all entities entering into service agreements with IPS. Form A requires disclosure of any relationships by blood or marriage with IPS Board members or staff involved in procurement, administration, or payment review. Form B addresses diverse and local supplier utilization, requiring contractors to identify whether they hold MBE, WBE, VBE, DOBE, or Local Business certifications and to demonstrate good faith efforts to include such certified businesses as subcontractors or suppliers. Form C requires letters of intent from minority and women-owned, veteran-owned, disability-owned, and local businesses that will participate in the project, confirming their certification status and proposed scope of work. The forms apply to the High Bandwidth Metropolitan Area Network (MAN) Services procurement, an e-rate funded telecommunications solution being solicited by IPS.
Forms D and E establish critical compliance certifications that contractors must complete prior to contract award. Form D requires E-Verify certification, mandating that contractors verify employment eligibility for all newly hired employees and certify they do not knowingly employ unauthorized aliens, with subcontractors required to provide identical certifications. Form E requires certification that the contractor and its principals are not suspended or debarred from federal contracting. Additionally, all contractors or individuals with direct or ongoing contact with IPS students must submit current expanded criminal history checks and Child Protection Index checks meeting the requirements of Indiana House Enrolled Act 1462, completed within ninety days of submission. The contractor bears all costs associated with obtaining these background checks. Failure to complete any required disclosure, certification, or background check documentation is sufficient cause for immediate contract termination.

Effective July 1, 2009, House Enrolled Act 1462 passed by the Indiana General Assembly requires that a current expanded criminal history check must be filed with the Board for each Contractor and Contractor employee that is likely to have direct or ongoing contact with IPS students.
An expanded criminal history check as defined by House Enrolled Act 1462, is a criminal history background check that includes a:

Contractors and their legal counsel shall review the requirements of House Enrolled Act 1462 (which defines an expanded criminal history check and modifies Indiana Code 2026-5-10) and file with the Board a current (no older than 90 days) expanded criminal history report meeting the requirements of House Enrolled Act 1462 for each Contractor, Contractor employee, or any person engaged by the Contractor who will have direct or on-going contact with IPS students during its course of work for the Board.
It is the full responsibility of the Contractor to obtain and file the expanded criminal history checks meeting the requirements of House Enrolled Act 1462 for every individual that will provide services and have direct or on-going contact with IPS students during the Contractor’s course of work for the Board.
Contractor will pay all expenses related to obtaining the expanded criminal history check.
