b. Delete the Performance Work Statement (PWS) and Evaluation Factors and replace with attached PWS and Evaluation Factors, changes are highlighted (if visible).
c. No more questions will be addressed or accepted after this amendment. The due date to submit offers is extended to read 12 Oct 2018; 1:00 PM PST instead of 09 Oct 2018.
d. All other terms and condition remain unchanged.

Q1-Per PWS 1.2, is the contractor required to provide continuous coverage at the Medical Center from 3:30 p.m. to 8:00 a.m.?
A1-Yes
Q2- Does PWS 3.5, Clean Interior Glass/Mirrors, also include interior surfaces of windows below 7 feet?
A2-Yes
Q3-Is there a requirement for exterior window cleaning?
A3-No
Q4-PWS Paragraph 4.1, Clean and Disinfect (Restrooms/Locker Rooms) states that the hospital will provide an EPA approved Hospital Grade Germicidal Detergent. Will the hospital provide the same or similar detergent for other tasks, such as PWS 3.4, Clean and Disinfect (surfaces and receptacles)?
A4-Hospital will supply all the cleaning supplies to the contractor. Refer to PWS paragraph 9.0 -Government-furnished items/supplies.
Q5-Per PWS 5.0, please provide required frequencies for: PWS 5.1, Strip, Scrub, Seal, and Wax Floors; PWS 5.2, Clean Interior Windows; and PWS 5.3, Shampoo Carpets.
A5- PSW 5.1 Quarterly or as needed; PWS 5.2 Weekly, PWS 5.3 as needed
Q6-Per PWS 6.0, Emergency or Special Event Cleaning Services, would the VA please provide data on these services, such as the number of emergency and special events and the approximate labor hours required.
A6-Emergency situations are unannounced events and there is no data to trend. As for special events EMS received a calendar for the medical center monthly.
Q7-Per PWS 11.8, will the hospital/VA provide radiation safety training?
A7-Yes
Q8- Please clarify the HAMS services requirements referred to in PWS 12.8 and identify areas for HAMS services in Attachment 3.
A8-See attachment 3.
Q9-Per PWS 12.12, please confirm if a HAMS Policy and Procedure Manual is required to be submitted with the technical proposal. Please confirm if this contract is part of the HAMS program.
A9- Yes- submit with the technical proposal.
Q10-PWS 12.1 through 12.5 describe the contents of the Contractor Quality Control Program with respect to an inspection system. PWS 12.6 through 12.12 describe a Contractor Quality Control Program with emphasis on work instructions/procedures. Please clarify.
A10- Contractor must provide a Quality Control Program within the Quality Control Program must be and Inspection Program. The Inspection Program must monitor the performance of their staff.
Q11-Per PWS 15.2, will the VA/hospital provide Red Bags and Sharps containers.
A11-Yes. Refer to PWS paragraph 9.0 -Government-furnished items/supplies.
Q12-Is a resume for the Executive Housekeeper required with the proposal?
A12-Yes Q13- Attachment 1, Buildings/Facility Listing Chart does not provide the square footage of the hallways/corridors for Buildings 3 or 210 and those building areas are not included on this list at all. Please provide the square footage of the hallways/corridors for those 2 buildings.
A13-Bldg. 7 includes connection to bldg. 2 and 210; Bldg. 2 includes connection to bldg. 3. Refer to Amendment 0002 Q&A#27.
Q14. Section 5.2 of the PWS says that the contractor will clean interior windows but the VA usually does a separate contract for cleaning interior and exterior windows. As interior windows are different from Section 3.5 of the PWS (interior glass, including mirrors/partitions/walls/display cases/directory boards), please verify whether the janitorial contractor is expected to clean the interior windows of the buildings. If so, please provide a count/number of those windows and their sizes for all buildings as this information is vital to correctly pricing this contract effort.
A14-The contractor is responsible for cleaning all entry glass doors and windows, interior glass windows up to seven feet in the Canteen. Hallway and corridor windows up to seven feet.
Q15-What are the requirements for porter service?
A15-It is not part of the contract
Q16-On the walk through it was mentioned that building 8 and 9 are not serviced, Can you confirm this?
A16- It was bldg. 9 & 10 (hoptel) and they are services during the day but left in the contract as possible delivery of linen or if there is any kind of spill that needs cleaning afterhours.
Q17-Is there any numbers for people or visits or patients for the Emergency Department?
A17-21,900 visits per year
Q18-Approximately how many services will be in the Operating Room?
A18-There are surgical specialties and dental surgeries that use the OR suites, i.e., general surgery, vascular, cardiology, eye, plastics etc.
Q19-Provided with the above numbered RFP there are two files containing US DOL Wage Determination
1. W/D # 1974-1257 (rev-41),, dated 12/26/17
1. W/D # 2015-5637 (rev-10), dated 7/3/18 These two W/D has variable wages and fringes which W/D should be used for costing this project.
A19- Delete the WD 1974-1257 rev 41 dated 12-26-2017 -1 and WD 15-5637 rev 10 dated 07-03-2018 and replace by WD 1974-1257 Rev 42 dated 10/01/2018.
Q20-The janitorial day shift (7:30 AM to 3:30 PM) will be staffed by VA personnel. Can we expect to find that facilities will have been serviced during the day to VA standard when we take responsibility at 3:30 PM?
A20- Yes Q21-We noted that two wage determinations were provided with the solicitation. 1) WD 1974-1257 (text file) and 2) WD 2015-5637 (PDF file). The rates differ. Which WD is operative?
A20- refer to A19.
Changes in PWS-
1- Under Parag 1.0 change the square footage to read 270,184 sq ft.
2- Add parag 3.11. Floods: Responsible for all floods from broken pipes, ceiling leaks, over flowing toilets and any spillage that happens during their tour of duty.
3- Under parag 5.1 delete the word “necessary” and change to read “quarterly or as needed” 4- Under parag 5.2 add the word “weekly” after the word surfaces.
5- Under parag 5.3 add the words “as needed” after the word cleaned.
6- Under Attachment 1- delete Bldg 211 and 2,117 square footage in its entirety. Change the total square footage to read “270,184”.
7- Under Attachment 2- change the frequency of the following to read- Conference room- Daily or more as needed Carpets/Vacuum- Daily or more as needed Hard Surface Floor - Daily or more as needed 8- Under Attachment 3-Catergory II, Patient Waiting Room add Saturday and Sunday (S) by X.

a. Under parag 1.2 add the words “provide resume”.
b. Add parag 1.4 to read –Contractor shall provide copy of their Housekeeping Aid Management Service (HAMS) Policy and Procedure Manual.
