About this file
This is a Statement of Work (SOW) for floor mat services for the Gulf Coast Veterans Health Care System (GCVHCS) in Biloxi and Mobile, VA. The contract requires a contractor to provide, deliver, and maintain high-quality floor mats with specific requirements: mats must be steel gray or black, heavy-duty, with beveled edges, manufactured with steadfast dyes, and capable of withstanding medical facility traffic including wheelchair and hospital bed movement. Delivery and pickup will occur weekly on designated days (Monday-Wednesday for Biloxi VA, Thursday for Mobile VA), with a 50% mat exchange each week.
Key contract terms include a 15% quantity variation allowance, replacement cost provisions, and strict quality standards. Contractors must comply with VA identification badge requirements, with personnel required to obtain photo badges before commencing work. Pricing will be determined by weekly rental fees calculated by multiplying quantity of each mat type by the contractor's unit price. The contract allows for potential quantity adjustments through bilateral modifications, with pricing based on the contractor's originally proposed mat listing prices for each location.
