SPECIAL NOTE
This Request for Quote (RFQ) solicitation is issued under the authority of FAR Part 13 Soliciting Competition, FAR Part 12 Acquisition of Commercial Items.
BASIS OF AWARD
The VA will select the quote that represents the best value to the Government at a price that can be determined fair and reasonable. This best value determination is ultimately based on the judgment of the contracting officer, but that judgement may include input and/or assistance from the VA end user(s) and/or technical expert(s). The VA will evaluate quotes using the following factors:
(1) Technical capability: ability to meet minimum performance requirements set forth in this document, while taking into account any value-added features or functionality above and beyond
(2) Training: proposed training plan offered to both clinical users and technical support providers at the VA
(3) Price; and
(4) Warranty & Maintenance: including depth of support model and documentation for system components/configuration and all troubleshooting education
After the solicitation closes, the Government will perform a comparative evaluation of the eligible quotes. All quotes must meet the minimum standards outline in this solicitation. For quotes that meet the minimum standards, the Government will perform a comparative evaluation by comparing quotes against one another to select the quote that best benefits the Government considering all factors. The VA will compare quotes against each other in an impartial and simplified manner based on FAR Part 13. Source selection techniques described in FAR part 15 will not be used.
Quotes deemed too expensive or not practical for the Government may not be considered further and without notice to the submitter. The Government may reject all quotes.
The contracting officer and/or VA user(s) may use any means available to examine quotes when necessary, to include but not limited to VA contracting officer and/or VA user’s knowledge of Contractor reliability, reputation, and suitability of the product or service to meet the VA’s need.
AUTHORIZED LIMITS
The Government estimates, but does not guarantee, that the IDIQ orders placed against this agreement will have an aggregate minimum of at least $189,307.00. This order limit may be increased by agreement of the parties as necessary, in whole or part. The authorized ceiling limit of the IDIQ is set at $2,500,000.00 over the period of the IDIQ. This ceiling limit is also not a guarantee. All orders including those that exceed the ceiling limits must be coordinated through the Contracting Officer before larger valued orders can be issued and prior to commencement of work. All unauthorized work, regardless of amount, will be processed through the ratification process.

Total Price, 5-year Ordering Period IDIQ:
NOTE: Since this IDIQ is estimated to exceed the simplified acquisition threshold, the Government is requesting that contractors offer a price reduction.
NOTE: Memo or correspondence which authorizes distributor to distribute the manufacturer’s products must be included with RFQ response if offeror is not original manufacturer. See 52.212-1, below.
OBJECTIVE
The Milwaukee VA Medical Center requires a new video and image management and archive system for its surgical department that will be 100% compatible with the existing video and image management technology already in use in operation rooms 1 and 8.
DESCRIPTION OF TECHNOLOGY REQUIREMENT
BROAD SUMMARY: The video and image management system shall provide the surgical staff with the ability to route and display video and images captured from various imaging modalities to multiple visual displays both within and outside of the OR environment, including the ability to live stream real time events. The solution shall be capable of supporting 4K video and image management, from the point of capture to display WITHOUT downscaling/upscaling the image data. The solution shall provide video/image data editing, archiving, and storage management that interfaces with the VA’s electronic health record (EHR) system, including the capability to receive HL7 orders from and sending HL7 results back to the EHR.
VIDEO & IMAGE MANAGEMENT: The video/image capture quality shall be 1080p high definition resolution or 4K, with the ability to support both single and dual channel recording. The 4K video/image data shall be unadulterated from point of capture to point of display (no downscaling or upconverting resolutions). Additionally, the solution shall provide live streaming functionality for staff to view procedures outside the operating environment.
The solution shall provide users the ability to edit video and image data, including the functionality to capture an image from recorded video for separate archiving and storage. The solution shall also feature patient privacy features that allow users to sanitize any video or image of all patient identifiable information so that it may be viewed or shared without infringing upon patient privacy policy and rights.
INTERFACES: The solution shall be vendor neutral such that it may accept video and image data from any device (i.e. microscopes, ultrasounds, C-arms, etc.), regardless of manufacturer, with the ability to route, capture, store, and transmit into the VA’s EHR system. The solution shall provide automatic video/image source identification within the graphical user interface, which alerts clinical staff to exactly what video sources are powered on and correctly connected to the network. The solution shall also incorporate the use of universal connection plugs that allow clinical staff to plug in any video/image capture device into any input receptacle, regardless of modality or system type.
The solution shall be specifically compatible with the following existing and/or frequently used equipment in the ORs: Stryker Equipment Delivery System and Booms, Visum In-Light cameras, Stryker 1288HD control units, Karl Storz Image 1S Connect control units, Olympus Evis Exera III and Boston Scientfici LithoVue Touch systems. The solution shall also interface with and control the existing surgical lighting and ambient lighting within the ORs.
The solution shall provide full HL7 integration with the EHR and query/retrieve integrating with the existing VA Milwaukee PACS systems via a compliant DICOM interface. This shall be done with a single point of integration, such that all video and image transmission from any OR pass across the same singular integration point between the image management solution and VA information systems.
The solution shall support VHA’s personal identification verification (PIV) card security measures. This interface will allow VA users to sign on/in into the image management system via established PIV card authentication.
USER INTERACTIONS: The solution shall provide a single graphical user interface (GUI) in each operating room.
