1.0 INTRODUCTION & EXECUTIVE SUMMARY
Mabbett and Associates, Inc. (M&A), with O’Brien & Gere, Inc. as a sub-contractor performed surveys for suspect asbestos containing building materials (ACM). Surveys were performed of selected buildings at the VAMC located at 400 Fort Hill Avenue in Canandaigua, NY, under Contract VA528-P-0738. A complete list of buildings surveyed is in Volume I of this report. Site survey work was performed during June 2010, by appropriately credentialed personnel.
Collected suspect ACM bulk samples were submitted to a certified analytical laboratory for asbestos analysis by polarized light microscopy (PLM) using stop positive methodology and transmission electron microscopy (TEM) as required by New York State Environmental Laboratory Analysis Program (ELAP). The survey effort involved the collection of the following samples, resulting in the following conclusions:
• 137 bulk samples for suspect ACM were collected in this building.
• Based on laboratory analysis 18 of the collected and analyzed samples contained asbestos greater than 1%. 1 homogeneous material was presumed ACM (PACM) based upon the VISN 2 sample protocol.
• 19 homogeneous areas have been identified to be ACM. These areas were presumed, previously identified, and/or have been identified during this survey to be ACM.
• The identified ACMs were generally limited to floor tiles, floor tile mastics, and previously identified thermal system insulation. A summary including a condition assessment, material description, and quantities can be found in Table 2 and drawings identifying the approximate locations and quantities of the ACM can be found in the Figures portion of this Volume II Report.
• The ACMs have been incorporated into the O&M Plan and can be found in
Volume IV of the Comprehensive VAMC Asbestos Survey Report. The O&M Plan provides detailed procedures to safely manage the asbestos that will remain in place.
• Based on the findings of this survey, M&A has not identified significantly damaged ACMs in this building that are recommended for abatement or immediate repair.
The purpose of this survey was to identify ACM and evaluate the associated potential risk of identified ACM to staff, patients and visitors. While the information contained in this report maybe useful for planning future renovation projects it is not the intent of this survey to identify ACM in inaccessible areas requiring destructive sampling or to be the sole basis for specifications for abatement, renovation, or demolition projects.
This building report consists of a summary of findings, floor plans indicating positive homogeneous areas, and detailed analytical findings for the specific surveyed building materials. Laboratory certificates of analysis for this building report are available in Volume III of the Comprehensive VAMC Asbestos Survey Report.

2.0 BUILDING DESCRIPTION
According to information provided by the VAMC Office of Facilities Management and observations made by the M&A team at the time of the survey, Building 36 was a four-story Day Treatment Center and Canandaigua School Building built in 1944 and occupied approximately 72,552 square feet.
3.0 ASBESTOS SURVEY
3.1 Sampling Methodology
A visual screening inspection was conducted by state licensed asbestos inspectors throughout the building to identify locations of suspect ACM. Only areas that were accessible during the field work phase were inspected. Every effort was made during the initial field survey work to access areas as necessary to complete the survey. However, if any areas remained inaccessible to the survey team, they are indicated in Table 1 below:
Table 1 – Specific Inaccessible Areas Canandaigua VA Medical Center, Building 36
Building Floor Room No. Reason Area Was Inaccessible and Survey Impacts, If Applicable
There were no inaccessible areas identified during this survey.
Other general areas that were inaccessible or where destructive sampling would be required and were therefore not inspected/sampled are identified below:
• Within walls • Within fire doors
• Enclosed pipe/duct chases • Inside mechanical equipment/ductwork
• Above fixed drywall or plaster ceilings • Intact ceramic tile, grout, and mastic
• Intact Terrazzo floor tile and mastic • Intact laboratory bench tops
• Roofs, associated materials, and equipment that may void roof warranties
ACM surveys should be performed prior to any proposed renovations or maintenance of areas and materials identified above and the areas and suspect materials should be presumed ACM unless proven otherwise.
Bulk samples were collected of suspect ACM in accordance with US Environmental Protection Agency (EPA) Asbestos Hazard and Emergency Response Act (AHERA) and the M&A Team Program Standard Operating Procedure (SOP), dated June 2, 2010. Bulk suspect ACM sampling was conducted according to the following sampling plan:
(a) Surfacing material:
(1) At least three bulk samples shall be collected from each homogeneous area that is
©2010, Mabbett & Associates, Inc. Page 3 of 8 Vol II Survey Rpt Canadaigua Bldg-36 (11-16-10)
1,000 ft2 or less.
(2) At least five bulk samples shall be collected from each homogeneous area that is greater than 1,000 ft2 but less than or equal to 5,000 ft2.
(3) At least seven bulk samples shall be collected from each homogeneous area that is greater than 5,000 ft2.
(b) Thermal system insulation:
(1) At least three bulk samples shall be collected from each homogeneous area of thermal system insulation.
(2) At least one bulk sample shall be collected from each homogeneous area of patched area of thermal system insulation.
(3) Sufficient samples shall be collected from elbows and fittings to determine if it contains ACM.
(4) Bulk samples shall not be collected from any homogeneous area where the state licensed asbestos inspector determined that the thermal system insulation is fiberglass, foam glass, rubber, or other non suspect material.
(c) Miscellaneous material:
(1) At least one bulk sample shall be collected from each homogeneous area that is less than 100 ft2.
(2) At least three bulk samples shall be collected from each homogeneous area that is greater than 100 ft2.
To limit the risk of exposure to ACM and to control costs, where previous surveys, labeling, and/or associated analytical reports indicated a material had been previously determined to be ACM, with a reasonable degree of accuracy and confidence by the inspector the material was generally NOT re-sampled. These materials are included in Table 2 and on the CADD plans of this building report. Associated previous laboratory results and/or supporting documentation are available in Attachment A of the Volume I portion of the comprehensive report.
3.2 Analytical Methodology
The collected bulk samples were submitted under chain of custody procedures to America Science, Inc. (Amerisci) of New York, NY for polarized light microscopy (PLM) and transmission electron microscopy (TEM) analysis as required by the New York State Environmental Laboratory Approval Program (ELAP). PLM analysis of friable materials was performed using NYS ELAP Method 198.1. Samples of Non-friable Organically Bound (NOB) materials were analyzed by PLM (NYS ELAP Method 198.6) subsequent to matrix reduction.
NOB samples that were identified as non-ACM by PLM methods were re-analyzed by TEM (NYS ELAP Method 198.4) for confirmation purposes. Amerisci is accredited by the American Industrial Hygiene Association (AIHA) and participates in the National Voluntary Laboratory Accreditation Program (NVLAP Lab Code 200546-0). Copies of the laboratory accreditations are included in Volume III of the Comprehensive VAMC Asbestos Survey Report.
Bulk sample results including positive and negative results are summarized in Table 3 – Summary of ACM Building Results. The EPA, Occupational Safety and Health Administration (OSHA), and Part 56 of Title 12 of the Official Compilation of Codes, Rules and Regulations of the State of New York (12 NYCRR Part 56) defines any material that contains greater than one percent (>1%) asbestos as being an ACM. As such, the analytical laboratory identified bulk
©2010, Mabbett & Associates, Inc. Page 4 of 8 Vol II Survey Rpt Canadaigua Bldg-36 (11-16-10) samples as positive for asbestos that met the regulatory criteria of greater than one percent (>1%) asbestos.
3.3 Summary of Asbestos Containing Material Findings
3.3.1 Data Tables and Laboratory Results
Collected bulk samples confirmed by the analytical laboratory to contain >1% asbestos, previously identified ACM and any PACM are listed in Table 2 - Summary of Positive Asbestos Containing Materials. An analytical laboratory summary for each of the collected bulk samples is listed in Table 3 - Summary of ACM Building Results. Volume III of the Comprehensive VAMC Asbestos Survey Report includes copies of the laboratory certificates of analysis.

* The VISN 2 Risk Hazard Assessment Scale 1 – 4 is a relative indicator of the risk and need for response/abatement. The rating assigned by an Asbestos Management Planner, takes into account: condition, friable vs. non-friable, accessibility, occupancy (e.g. continuous, intermittent or occasional and patients/staff/visitors), potential for air erosion, potential for
©2010, Mabbett & Associates, Inc. Page 7 of 8 Vol II Survey Rpt Canadaigua Bldg-36 (11-16-10) vibration damage, potential for disturbance / damage (e.g. exposed and in an accessible location), and potential for water damage. The following is a list of the VISN 2 Risk Hazard Assessment Scale:
Category 1: represents significantly damaged friable ACM in locations with moderate to high occupancy by staff and/or patients. A significantly damaged condition is a condition with damage greater than or equal to 10% of the total surface area or 25% localized damage. Category 1 ACM should be abated or repaired as soon as possible to minimize any potential hazards posed by the asbestos.
Category 2: represents significantly damaged and damaged friable ACM in locations not frequently occupied. Category 2 ACM should be abated or repaired as soon as possible to minimize any potential hazards posed by the asbestos.
Category 3: represents ACM in at least good condition (friable or non-friable ACM that could become friable). A good condition is a condition with damage less than or equal to 1% of its total area. Category 3 should be periodically visually surveyed semi-annually as part of the routine O&M Plan and should be considered for abatement or corrective action if conditions deteriorate or circumstances allow.
Category 4: represents non-friable ACM in good condition but should be regularly inspected as part of the O&M Plan.
3.3.2 CADD Drawings
The approximate location of each bulk sample and its associated sample ID are shown on the CADD drawings in the Figures Section of this report. Each positive ACM sample location is colored red and marked with an asterisk (*). Areas containing ACM have been indicated with hatching to identify the locations of the identified ACM. Previously identified ACM not sampled during this survey are also included on the drawings.
ACM abatement or newly identified ACM should be annotated on the CADD drawings by the VAMC going forward.
4.0 LIMITATIONS
This inspection report is the result of a diligent search of the building for ACM. Only accessible areas were included in this survey. However comprehensive this inspection appears, it does not claim to have identified all of the ACM that could be present in the facility. M&A’s survey was performed with limitations inherent to visual non-destructive inspections. M&A has conducted this assessment with reasonable care and has performed this project within generally accepted industry standards. There can be no assurances, and M&A makes no assurances, that the information, research, and technology used to prepare this report may not change in the future, thus affecting the results provided. The report is designed to aid the building owner, architect, construction manager, general contractors, and potential asbestos abatement contractors in locating ACM. Under no circumstances is the report to be utilized as a bidding document or as a project specification document since it does not have all the components required to serve as an Asbestos Project Design document or an Abatement

Workplan. Prior to renovation, the collection of additional samples may be required of inaccessible areas such as wall cavities, pipe chases, etc.
5.0 CLOSING REMARKS
Prior to initiating plans for maintenance, renovation or demolition activities, the VAMC should review the asbestos survey results to identify if any of these materials will be disturbed by proposed work activities.
The purpose of the ACM survey was to identify ACM in the building within the limitations of the survey for worker and patient protection purposes. In regards to asbestos, any suspect material encountered or expected to be encountered during renovation/demolition that is not identified in this report as being non-ACM should be assumed to be ACM unless sample results prove otherwise. Suspect ACM that may be present within the walls, above inaccessible hard ceilings, or in other inaccessible locations, that were not inspected should be assumed to contain asbestos if discovered during any renovation process until verified otherwise.
If ACM must be removed, the ACM must be removed by a licensed asbestos abatement contractor and be performed in accordance with the applicable regulations. If proposed work activities will not disturb ACM, continuous monitoring of ACM should be conducted throughout work activities to ensure the ACM remains in an intact condition. Additionally, prior to commencing work activities, VA employees and contractors involved with the work activities should be made aware of the location of ACM, within the building in which they will be working.
Minor repairs to ACM which may result in the disturbance of ACM must only be performed by trained and qualified VAMC staff or contractors as outlined in the O&M Plan. Additional information regarding asbestos management and the Operations & Maintenance (O&M) Plan is outlined in the VAMC campus wide Asbestos O&M Plan available in Volume IV of the Comprehensive VAMC Asbestos Survey Report.
